Commonwealth of Pennsylvania by its solicitor, Martson Deardorff Williams Otto Gilroy &
Faller, hereby files its claim for sewer services against the real estate hereinafter described
and sets forth its claim as follows:
STATEMENT OF CLAIM

The name of the municipality by which this claim is filed is North Middleton Authority, a
municipal authority of the Commonwealth of Pennsylvania, located in the County of
Cumberland and organized pursuant to ordinances of North Middleton Township.

The name or names of owner or owners or reputed owner or owners, of the property against
which this claim is filed are' David D. Myers and Amy S. Myers of 911 West North Street,
Carlisle, Pennsylvania.

A description of the property against which this lien is filed is attached hereto and marked as
Exhibit "A" with an address of 911 West North Street, Carlisle, Pennsylvania.

The authorities under and by virtue of which this lien is filed is the Municipal Claims and
Liens Act of May 16, 1923, P.L. 207, as amended and supplemented, 53 P.S. 7101 et seq.

The lien is claimed for unpaid sewer user fees against the above-described property in the
amount of $4,700.00.

Interest is claimed on the above-mentioned amount from October 26, 2006.

DEED MADE THE 15' day of AUGUST, 2005.
BETWEEN BRIAN K. MITTEN and MIRIAM W. MITTEN, husband and wife, of
Cumberland County, Pennsylvania, hereinafter referred to as:
Grantors,
and DAVID D. MYERS and AMY S. MYERS, husband and wife, of Cumberland County,
Pennsylvania, hereinafter referred to as:
Grantees,
In consideration of ONE HUNDRED THIRTY SEVEN THOUSAND FOUR HUNDRED
AND 00/100 ($137,400.00) DOLLARS, the receipt whereof is hereby acknowledged, the Grantors
do hereby grant and convey to the Grantees, their heirs and assigns:
ALL THOSE two (2) certain lots of ground, together with the improvements thereon
erected, situate in North Middleton Township, Cumberland County, Pennsylvania,
bounded and described as follows:
BOUNDED on the South by West North Street; on the West by Lot No. 21 on the
hereinafter mentioned plan of lots; on the North by Lot No. 11 on the hereinafter
mentioned plan of lots; and on the East by Lot No. 18 on the hereinafter mentioned
plan of lots. The said two (2) lots being 100 feet in width along the said West North
Street, and extending at an even depth of 200 feet. Having thereon erected a one-
story ranch type dwelling house known and numbered as 911 West North Street,
Carlisle, Pennsylvania.
BEING Lot Nos. 19 and 20 on the Plan of Lots known as "Burr Heights
Development", said plan being recorded in the Office of the Recorder of Deeds in
and for Cumberland County, Pennsylvania, in Plan Book 5, Page 47.
THIS conveyance is made under and subject to all the building and other restrictions
attached to the aforesaid plan of lots.
BEING the same premises which Robert E. Wilfong and Miriam H. Wilfong,

husband and wife, by their deed dated November 6, 1991, and recorded in
Cumberland County, Pennsylvania Deed Book "S", Volume 35, Page 665, granted
and conveyed unto Brian K. Mitten and Miriam W. Mitten, husband and wife,
Grantors herein.
AND the said Grantors hereby covenant and agree that they will warrant specially the
property hereby conveyed.
IN WITNESS WHEREOF, said Grantors have hereunto set their hands and seals the day and
year first above written.
