1. Minor child is Kayla M. Yuda, who resides at 1155 Baish Road, Mec~nicsburg,
Pennsylvania, 17055, and who was born on April 18, 2001. The names and address of the
parents are as follows: Owen G. Yuda, 1155 Baish Road, Mechanicsburg, Pennsylvania,
17055, and April Rose, 931 Harrell Avenue, Frostproof, Florida, 33843.
2. The petitioner is the natural father of the child and the petitioner resides with the
child.
3. The consents of the minor child's mother and Jessica L. Barnhart are attached
hereto for the proposed guardian, Jessica L. Barnhart, age 24, who resides with, and who is
the girlfriend of the natural father.
4. The appointment of the proposed guardian is necessary because Father is often
unable to handle school matters and/or medical issues and it is desired that the proposed
guardian be permitted to transact these matters for the child. Additionally, Jessica L. Barnhart
will be permitted to add the child to Ms. Barnhart's health insurance if this guardianship is
permitted.
5. The proposed guardian has no adverse interest to that of the minor child.

6. The religious persuasion of the parent is Catholic and the proposed guardian is
Lutheran. The proposed guardian's written consent to the appointment as guardian is
attached hereto.
