PETITION FOR CHANGE OF NAME
The petitioner Peter Wasch, through his undersigned counsel, request that this
Court order a change of name for him, pursuant to 54 Pa. Cons. Stat. Ann. § 701 et seq.,
and in support thereof represent as follows:
Petitioner Peter Wasch is a resident of 1493 Brandt Ave. New Cumberland,
Pennsylvania, and has resides at the following location during the five years next
preceding the filing of this petition.
Petitioner Peter Wasch is an adoptee having been bom in Harrisburg Hospital on
July 1, 1978, as Peter Konstantinos Koutsikos.
Petitioner Peter Konstantinos Koutsikos was adopted by William Wasch and Kiki
Wasch on September 30th, 1991, at which his name was changed to Peter Wasch.
Because petitioner Peter Wasch wishes that his name reflect his true identity and
heritage and because of his desire to have any children or other descendents bear his
family name, petitioner desires to be known as Panagiofis Konstantinos Koutsikos.
WHEREFORE, petitioner requests that this Court enter a decree changing his
name from Peter Wasch to Panagiotis Konstantinos Komsikos.

In accordance with the Name Change Act of 1998, the fingerprint cards of the above named individual, who
has applied for a name change, has been searched in the files of the Pennsylvania State Police, Criminal Records and
Identification Division Central Repository, and:

CHANGE ACT, THE COURT SHALL NOTIFY THE PENNSYLVANIA STATE POLICE WHEN
A NAME CHANGE FOR A PERSON CONVICTED OF A FELONY HAS BEEN ORDERED. The
Pennsylvania State Police, upon receipt of this notice, shall include the change of name information in the
Central Repository as provided for in 18 PA C.S., Chapter 91 (relating to criminal history record
information).
