PETITION FOR NAME CHANGE
AND NOW comes the Petitioner, Steven Henry Erno, by and through his attorney,
Jane Adams Esquire, and respectfully requests this Honorable Court to change his name
to Melyssa Candace Morgan for the following reasons:
1. Petitioner, Steven Henry Erno, currently resides at 404 Coover St.,
Mechanicsburg, Cumberland County, Pa. 17055.
2. Petitioner, Steven Henry Emo, was born on April 9, 1955·
3. Petitioner has been a lifelong resident of Central Pennsylvania.
4. Petitioner is male but has been living as a woman since 1997.
5. Petitioner is planning on having a sex change operation within approximately one
year and his doctor requires that he have his name changed before the operation.
6. Petitioner has been consistently using the name Melyssa Candace Morgan
informally since 1997.
7. Petitioner has no outstanding credit debt that could be avoided through this name
change.
8. Petitioner has no outstanding federal or state tax debt that could be avoided through
a name change.
9. Petitioner has no knowledge of any company, person or relationship that will be
adversely affected by changing his name from Steven Henry Erno to Melyssa Candace
Morgan.
WHEREFORE, Petitioner respectfully requests your Honorable Court to grant the
foregoing Petition for Name Change and change his name to Melyssa Candace Morgan.
