Buckley, Esquire and petitions this Court as follows: 1. Petitioner Edward Gamet Alexander was born out of wedlock on December 12, 1964, in Carlisle, Cumberland County to Shirley A. Alexander and Ronald V. Piccolo. 2. As he was growing up, Petitioner was told of his natural father and his father's name; however, his birth certificate gave him the family name Alexander.(A true and correct copy of Petitioner's birth certificate is attached and Marked Exhibit "A", a true and correct copy of the front and back of Petitioner's Carlisle Hospital Birth Record is attached and marked Exhibit "B-1"and "B-2".) 3. On July 23, 1988, Petitioner Edward Garnet Alexander married Yvette Susan Alexander (nee Rotz). 4. Petitioners have resided for their entire lives in Cumberland County and has resided during the past eight (8) years at 14 East Eppley Drive (South Middleton Township), Carlisle, PA 17013. 5. Two children have been born of the Petitioners' union: Sean Michael Alexander (born 11/30/1994) and Shannon Marie Alexander (born 3/31/1997). 6. In 1999, Petitioner was successful in seeking out and meeting his natural father and his father's family in the Philadelphia area. 7. The family resemblance was very noticeable and Petitioners were invited and attended family functions and reunions. 8. Petitioners desire to assume the family name of Edward Garnet Alexander's natural father, that being, Piccolo. 9. Petitioners' request is motivated by their family connection to the Piccolos. 10. Petitioners' oldest child is soon to enter public school and Petitioners desire to have their names as well as their children's names changed before school begins. 11. Petitioner's only current debts are the mortgage on their home, said mortgage filed in the Recorder of Deeds Office, nominal credit card debt and two automobile loans. 12. Petitioners are not attempting to change their names for any other reason than those set forth in this petition. 13. Petitioners have traveled to the Pennsylvania State Police Barracks in Carlisle for the purpose of having their fmgerprints recorded and their respective fingerprint cards are attached and marked Exhibits "C-1" and "C-2 ; WHEREFORE, Petitioners pray this Honorable Court enter an Order changing their names from Edwazd Garnet Alexander to Edward Alexander Piccolo and from Yvette Susan Alexander to Yvette Susan Piccolo and their children's family name from Alexander to Piccolo.
