PETITION FOR CHANGE OF NAME
AND NOW, this ~ day of July, 2001, comes the above-named petitioner, Mira
Freml, by and through her attorney, Courtney L. Kishel, Esquire, and The Law Offices of
Richard C. Gaffney, who respectfully requests This Honorable Court to grant this petition
for change of name, pursuant to 54 Pa.C.S. §§701 and 702 and in support thereof avers
the following:
1. Petitioner, Mira Freml, is an adult individual residing at 313 South
York Street in Mechanicsburg, Cumberland County, Pennsylvania,
17055, with her husband, Stjepan Freml, and son, Vladimir Freml.
They have resided there since August 2000.
2. Prior to August 2000, Petitioner resided with her family at 1303 Loisa
Lane in Mechanicsburg, Cumberland County, Pennsylvania, 17055.
Petitioner and her family resided there from April 1999 to August
2000. Prior to that, Petitioner resided at 147 East Main Street in
Mechanicsburg, Cumberland County, Pennsylvania, 17055.
3. Petitioner has been a bona fide resident of the Commonwealth of
Pennsylvania for at least six (6) months prior to filing this petition.
2. Petitioner was born November 10, 1946, in Kosna, and given the name
Mira Serbula. (See Exhibit A).
Petitioner's husband was born January 1, 1944, in Matijevici, Croatia,
under the name Stjepan Zvonimir Fromml. (See Exhibit B).
4. Following the end of World War II, the occupying Communist
Government in Yugoslavia forced Mr. Freml to change his name from
Stjepan Zvonimir Fromml, to Stjepan Freml. (See Exhibit B).
5. On July 1, 1972, Petitioner married Stjepan Freml in Bosanski-
Herzegovina and took her husband's last name of "Freml".
6. On May 8, 1997, Petitioner and her husband became a permanent
residence of the United States. (See Exhibit C).
On October 25, 2000, the Croatian Government granted Petitioner's
husband's request to change his name from Stjepan Freml, to the
original spelling of Stjepan FrSmml. (See Exhibit D).
8. Petitioner hopes to obtain United States citizenship in October, 2002.
9. Petitioner wishes to obtain citizenship under her husband's original
name, rather than the name given to him by the communist
government, because it reflects her husband's family heritage.
10. Petitioner has one son, Vladimir Freml, who has also filed a petition to
change his name to the original family spelling.
11. Petitioner does not have any liens pending against her.
12. Petitioner is not attempting to avoid civil or criminal liabilities, nor is
she filing this petition for another other fraudulent purpose.
WHEREFORE, for the foregoing reasons, Petitioner, Mira Freml, respectfully
requests This Honorable Court to grant this petitioner to change her name from Mira
Freml, to Mira Serbula Fromml.

na Freml iz SAD, sa posljednjim prebivalis"tem u RH Rujevac 1R, Dvor, u
predmetu promjene prezimena, temeljem dlanka 6, stavak 3. Zakona o osob-
nom imenu ("Narodne novine", broj: 69/92), donosi
R J E S E N J E
Odobrava se Stiepanu Freml iz SAD, posljednje prebivali~te u RH Ru-
jevac iA, roHen 01.01.1944. godine u Matijevidi, Dvor, otac Ilija Freml,
majka Sarafina Freml r. Kujundzid, promjena prezimena iz Freml u "Fromml"
Po pravomodnosti rjesenja nalaz"e se nadle~nom maticaru izvr~enje
upravne radnje naknadnoa upisa odobrene promjene prezimena u drzavnim
maticama.
O b r a z l o~ e n j e
Stjepan Freml iz SAD, obratio se ovom Uredu putem Veleposlanstva
RH a SAD, Washington, Odjel za konzularne poslove, kojom trazi promjenu
prezimena iz Freml u "Fromml".
U zamolbi navodi da deli promijeniti prezime u Fromml jer je to o-
biteljsko prezime, a izmijenjeno je poslije drugoga svjetskog rata na-
kon smrti njeaovog oca Ilije Fromml, koji je poginuo 24.12.1944. Bodi-
ne u selu Struga, Dvor. Istic"e da je prezime tada promijenjeno u fonets-
ki oblik Freml koje je koristeno sve do sada. Prilaze vjencani list svo-
jih roditelja iz kojeg je vidljivo da je prezime "zenika i njeaovog oca
Fromml. Slijedom navedenog deli vratiti obiteljsko prezime Fromml.
Obavijest o promjeni prezimena objavljena je na oalasnoj ploci i na
nju nije bilo primjedbi.
U provederom postupku, utvrHeno je da nema zapreka iz c"lanka 8, 9.
i 10. stavak 3. Zakona o osobnom imenu, to je primjenom clanka 6. stavak
1. istoq Zakona, rije~eno kao u izreci.
Upravna pristojba na ovo rjedenje naplaEena je u iznosu od 70,OOKn
upravnih biljega po Tbr. 1. i 2. Zakona o upravnim pristojbama /"Narod-
ne novine", broj: 8/96).
UPUTA U PRAVNOM LIJEKU:
Protiv ovog rje~enja moz"e se izjaviti zalba u roku 15 dana od dana
primitka rje~enja Ministarstvu pravosuda, uprave i lokalne samouprave,
Zaareb, Republike Austrije 16, kao drugostupanjskom tijelu.

a upravna pristojba se placa u iznosu od Kn 50,00 upravnih biljega po

Freml from the USA, with the last residence in the Republic of Croatia at Rujevac lA,
Dvor, in the subject of surname change, on the basis of the Article 6, Item 3 of Personal
Name Law ("Narodne novine", number 69192), has made
The Decision
It is allowed to Stjepan Freml from the USA, with last residence in the Republic
of Croatia in Rujevac lA, born on January Ol, 1944, at Matijevici, Dvor; father IIija
Freml, mother Sarafina Freml nee. KujundCi6, the change of surname from Freml into
"Fromm!".
By the validation of the decision the authorized registrar is ordered to perform the
administrative action of subsequent registration of the allowed surname change in the
state register of births, marriages and deaths.
Explanation
Stjepan Freml from the USA has contacted this Office by the Embassy of the
Republic of Croatia in Washington Consulaz Affairs Department, with which he requires
the surname change from Freml into "Fromm!".
In the request he states that he wishes to change the surname to Fromm!, since it
is the family's surname, and had been changed after the World Waz Two following the
death of his father Ilija Fromm!, who was killed on December 24, 1944 at village of
Struga, Dvor. He emphasizes that the surname had been changed to phonetic form of
Freml at that occasion, and was used until now. He encloses the certificate of marriage of
his parents', which makes visible that the name of the groom and his father is Fromm!. In
sequence to the stated, he wishes to regain his family's surname of Fromm!.
The notice on surname change has been posted on the notice boazd and no
objections to it have been raised.
hY the procedure which was conducted, it has been established that no obstacles
from Articles 8, 9 and 10 item 3 of the Personal Name Law exist, and therefore by
application of the Article 6, item 1 of the Law, the decision has been made as in the
proposition.
LEGAL REDRESS ADVICE: -
The complaint can be filed against this decision within the course of 15 days of
the day of the receipt of it, to the legal body of second instance, the Ministry of Justice,
Administration and Local Self-Governing, in Zagreb, 16 Republike Austrije Street.

and recorded in protocol. Administration tax is to be paid in the amount of legal stamp's
