Petition for Name Change.
3. This Petititioner wishes to change her name formally to ,
Hannah Claire Weis for security reasons. The National Coalition
Against Family Violence reccommended that I come to Penn. State to
go through this final name and Socila Security change. The enclosed documents
will explain my particular situation better. And even though it does not fit
the usual peramiters for time span of residency, since the Federal Government
has given me -a new S.S. # as of this April, 2001, there is due cause for
asking the Court to honour this request. I will be willing to provide the
Judge will more information at the hearing, if necessary. Thank you for
understanding.
_ If this Petition is approved, the Petitioner intends to use the name
consistently, non-fraudently, and exclusively.
_ It is not the purpose of this Petition to avoid any taxes or any other debts.

ORDER
NOW ON THIS ~_ day of -; 2001, the above-styled
and numbered cause comes on for hearing before the undersigned Judge of the District
Court upon the verified Application of the Petitioner for change of name. The Petitioner
appears in person, and the Court, having heard the sworn testimony of the Petitioner,
FINDS:
1. That prior to filing this Application for Name Change, the Petitioner has been a
resident in good faith of Ottawa County and the State of Oklahoma for more than
thirty (30) days.
2. That the Petitioner desires that her name be changed from LINDA S. STARK to
SKYLER BETH 3ONES.
3. That pursuant to 12 O.S. § 1633 legal notice of the filing of the Application for Name
Change was published one time, at least ten (10) days prior to this hearing in a
newspaper of general circulation in Ottawa County, Oklahoma, authorized by law to
publish legal notices.
4. That the Petitioner, birth certificate #354, was born on Mazch 18, 1947, in
Brownwood, Brown County, Texas, and that her birth certificate is registered with the
County Clerk's office of Brown County, Texas.
5. That no protests to Petitioner's Application for Name Change aze filed herein.

6. That the Petitioner is entitled to the relief prayed for in her verified Petition for the
reasons set forth therein, and that the Petitioner does not seek to have her name
changed for any illegal or fraudulent purpose, or to delay or hinder creditors.
IT IS THEREFORE ORDERED, ADJUDGED AND DECREED by the Court
that the name of the Petitioner be and the same is hereby changed to SKYI.,ER BETH
JONES and that copies of this Order maybe furnished to the necessary agencies to make

Comes now, Skvler Beth Jones Pazker ,Petitioner herein, and on behalf of Skvler Beth
Jones Parker ,request that the Court consider his/her Petition for Order of Protection and after
consideration thereof enter and Ex-Parte Temporary Order of Protection on her behalf. The
Petitioner states that the Respondent has committed acts of domestic abuse against her and fears that
there is an immediate and present danger of future acts of abuse by the Respondent. The Petitioner
further states that:
1. That, Skyler Beth Jones Pazker, the petitioner is at 54 years of age.
2. That Jimrnie Nowell Pazker, the respondent is at least 18 yeazs of age.
3. The Petitioner and Respondent were married on June 20, 2001.
4. That the Respondent has committed domestic abuse to the Petitioner by the following acts:
There has not been any kind of phvsical abuse until the day we were married. then it all started.
I have written a statement at the sheriffs office that has all of the information.
5 That the Petitioner is afraid of the Respondent and feels that there is an immediate and present
danger ofdomestic abuse to her because: I feel that the only reason he wanted me to him was
that it was a business deal for him. then when he started taIkin¢ about how to nlan the perfect murder
I realized that everything_I had would be his if something happened to me I became afraid He then
started making it known to evervone even people in foreien countries that I had been through the
Witness Protection Program.
The Petitioner request that the Court issue and Ex-Pane Temporary Order of Protection and upon
final hearing issue a full Order of Protection with the following provisions:
