Plaintiff is a corporation with offices located at 323 Lakeside Avenue, Cleveland, Ohio 44113. 2. Defendant is an adult individual residing at 1004 E. Simpson Street, Mechanicsburg, Cumberland County, Pennsyvlania 17055-3465. 3. Defendant applied for and received a MASTERCARD credit card issued by Plaintiff `s assignor bearing the account number 5447-0500-0110-3428. 4. Defendant made use of said MASTERCARD credit card and has currently a balance due and owing to Plaintiff, as of April 13, 1998, in the amount of $1,303.89, as shown by Plaintiffs Statement of Account attached hereto, marked as Exhibit "1" and made a part hereof. 5. Subsequent to the referral of this account to Plaintiff's counsel by Plaintiff, the Defendant made four payments of $100.00, totaling $400.00. The payments were applied to the balance, therefore reducing the Defendant's balance to $1,369.62 as of May 11, 2001. 6. Defendant is in default of the parties' agreement having not made payment to Plaintiff as promised, thereby rendering the entire balance immediately due and payable. 7. Plaintiff avers that Plaintiff is entitled to the addition of interest at the rate of 10% per annum on the unpaid balance from May 11, 2001, which amounts to $.35 per diem. 8. Although repeatedly requested to do so by Plaintiff, Defendant has willfully failed and/or refused to pay the principal balance, interest or any part thereof to Plaintiff. WHEREFORE, Plaintiff demands Judgment in its favor and against Defendant, Linda L. Liddick individually, in the amount of $1,369.62 with continuing interest thereon at the rate of $.35 per diem plus costs.
