Gunderson, by and through her Attorney, Jane Adams, Esquire, and respectfully requests this Honorable Court to change her name to Meagan A. Rouse for the follov~ing reasons: 1. Petitioner, Meagan A. Gunderson, (hereinafter referred to as "Petitioner") currently lives at 4126 Kittatinny Drive, Mechanicsburg, Pennsylvania, 17055. 2. Petitioner's date of birth is March 4, 1987. 3. Petitioner lives with her natural mother, Nancy Rouse, and her step-father, William L. Putt. 4. Nancy Rouse and William L. Putt were married on December 31, 1999; Nancy Rouse chose to retain her maiden name after the marriage. 5. Petitioner's natural father, R. Scott Gunderson, is currently believed to reside at 468 Island Road, Bossier City, Louisiana, 71111, with his wife, Belinda Johnson. 6. Petitioner has very limited contact with her father, R. Scott Gunderson. 7. Petitioner desires to use her mother's maiden name since she resides with her mother. 8. Petitioner believes that using her mother's maiden name would avoid confusion regarding school and medical records. 9. Petitioner has no outstanding credit debt that could be avoided through a name change. 10. Petitioner has no outstanding federal or state tax debt that could be avoided through a name change. Scott Gunderson is currently avoiding service due to the fact that Louisiana Support Enforcement Services is currently attempting to serve a support Order upon him. 5. Pursuant to Pa.R.C.P. 430, Petitioner requests that an Order be entered allowing service by publication in Shreveport Times, which is a newpaper of general circulation in the county of R. GUNDERSON Petitioner NO.O1 - 5414 CIVIL ACTION -LAW AFFIDAVIT Affiant, Jane Adams, Esquire, hereby deposes and avers that pursuant to attempting service of the Petition upon R. Scott Gunderson, father of Meagan A. Gunderson, Petitioner in the above-captioned matter, the following actions were undertaken by affiant: 1. A copy of the Petition and Order setting a hearing date was forwazded to R. Scott Gunderson at his last known address, at: 4814 Jeb Stuart Place, Bossier City, Louisiana, 71112 via certified mail. As of this date, this item has not been claimed or returned by the post office. 2. A search for the address of Belinda Johnson, R. Scott Gunderson's paramour, was completed on the Internet utilizing anywho.com. The address of 468 Island Road, Bossier City, Louisiana, 71111 was obtained. A copy of the Petition and Order was sent to this address via certified mail. The mail was returned indicating there was no such street. (See Exhibit #1). 3. A phone call was made to the Post Office in Bossier City, Louisiana, telephone number 1-318-746-7926. An inquiry was made as to the sufficiency of the previous address. Affiant was notified that there was no such street in Bossier City, however that there may be an Island Road in Shreveport, Louisiana. 4. A copy of the Petition and Order was sent to 468 Island Road, Shreveport, Louisiana, 71105. The mail was returned labeled "insufficient address." 5. Affiant called Libby Hapgood, casemanager for Louisiana Support Enforcement Services, P.O. Box 18590, Shreveport, Louisiana, 71138-1590, telephone number 1-318-676- 7010. Ms. Hapgood indicated that she was attempting to serve R. Scott Gunderson with a support enforcement Order. A copy of the above-captioned Petition and Order were sent to Ms. Hapgood in order that the Sheriff could serve all documents contemporaneously. As of the current date, Ms. Hapgood has not been able to locate R. Scott Gunderson at any of the above- addresses or any other address and the documents are heretofore unserved.
