MECHANICS' LIEN CLAIM
1. Claimant is Mellon Certified Restoration hereinafter referred to as "Claimant".
2. Defendant/Owner is John L. and Christine D. Capps, who resides at 923 Brandywine
Way, Mechanicsburg, Pa. 17050 hcreinatter referred to as "Defendant/Owner".
3. The property of the Defendant/Owner that is subject to the mechanics' lien claim by the
Claimant is the property and improvements thereon located at and commonly ]mown as
923 Brandywine Way, Mechanicsburg, Pa. 17050.
4. On or about May 23, 2001, the Claimant had completed the restoration as a building
contractor in connection with the water loss at the home at the aforementioned property
wvned by the Defendant/Owner.
5. Tne labor and materials subject [o a mechanics' lien claim pursuant to the Mechanics'
Lien Law, act of August 24, 1963, PL 1175, No. 497, Section 1503 (6); 49 P.S. Section
1503 (6) which were provided by Claimant are those described in the scope and itemized
pricing set forth from Claimant to DefendantlOwner which is attached hereto as Exhibits
"A" and"B".
6. As the consequence of the labor and materials tunushed by the Claimant, the Claimant
has not been paid and is owed the sum of $ 11,045.67, of which a copy of the customer
invoice is attached hereto as Exhibit "C" .
