It is hereby stipulated that:

Jean Renee Byrd, natural mother of herein named minor, is an
adult individual who currently resides at 1119 Redwood Dr., Carlisle,
Cumberland County, Pennsylvania 17013.
2. Michael Anthony Davis, natural father of herein named minor, is an
adult individual who currently resides at 612 Seneca St., Harrisburg,
PA 17101.
3. Miean Cheyanne Alyce Davis, minor and natural child of Jean R. Byrd
and Michael Anthony Davis, currently resides at 612 Seneca St.,
Harrisburg, PA 17101.
4. Jean Renee Byrd wishes to give custody of Miean Cheyanne Alyce
Davis to Michael Anthony Davis. Michael Anthony Davis will have full,
legal and physical custody of Miean Cheyanne Alyce Davis, including
the right and responsibility to make decisions regarding Miean
Cheyanne Alyce Davis' education, health, financial and any other
decision that may be required to properly care for said minor.
5. Michael Anthony Davis wishes to accept full, legal and physical
custody of Miean Cheyanne Alyce Davis.

6. There is no requirement for Jean Renee Byrd to pay child support in
any manner or form.
