Plaintiff is Nona I. Garner - Shedff, an adult individual whose residence is at 531 Conodoguinet Ave., Carlisle, Cumberland County, Pennsylvania. 2. Defendant is Roger Garner, an adult individual whose residence is at 424 Avenue North, Columbia, Pennsylvania 17512. 3. Defendant, Linda Helman, is an adult individual whose last known address is P.O. Box 1483, Lebanon, Pennsylvania 17042. 4. Plaintiff is the paternal grandmother and seeks custody of the child, Victoria Marie Gamer, born January 11, 1994, who currently resides with the paternal grandmother at the above referenced 531 Conodoguinet Ave., Carlisle, Pennsylvania 17013· 5. Since the birth of the child, the child has resided off and on with the grandmother and since 1999 at age five the child has resided constantly with the paternal grandmother, plaintiff herein. 6. The relationship of the Defendants to the child is that of natural father and natural mother· 7. The Defendants where never married to each other· 8. The Plaintiff has not participated as a party or in any other capacity, in other litigation concerning the custody of the child in this or any other Court. 9. It is believed and therefore it is advised that Defendant father consents to an order granting care and custody of his daughter Victoria Marie Garner, to the Plaintiff, paternal grandmother Nona I. Garner- Sheriff. 10. It is believed that the mother who is elusive and desires not to reveal her residential address, agrees and concurs that Nona I. Garner-Sheriff Plaintiff should have care and custody of her daughter, Victoria Marie Garner. 11. The Plaintiff, paternal grandmother, Nona I. Garner-Sheriff desires to have granddaughter maintain contact with both her natural father and mother; the life styles of both natural parents are not in the best interest of the child, Victoria Marie Garner. 12. The Plaintiff has been faced with situations involving medical care and educational decisions relative to the welfare of the child and some of the obstacles that she has met and expects to meet in the future are potentially destructive to the child's welfare absent a court order permitting custody to lie with the Plaintiff. 13. The best interest and permanent welfare of the child will be served by granting the relief requested. 14. Each parent whose parental rights 'to the child have not been terminated and the person who has physical custody of the child have been named as parties to this action. No other persons are known to have or claim to have any right to custody or visitation of the child other than the parties to this action.
