The Petitioner has resided in the Commonwealth of Pennsylvaula for the last five years as

If this Petition is approved, the Petitioner intends to use the name Andrew Yu-Jen Wang
consistently, non-fraudulently, and exclusively.
It is not the purpose of this Petition to avoid any taxes or any other debts.
