Petitioner, Lynsie Domene, is an adult individual residing at 66 Old Stonehouse Road, Carlisle, Cumberland County, PennsylvR."1ia 17013-9785. 2. Petitioner is the biological parent of one child, Jesse Robert Domene, a minor born out of wedlock on September 16, 1990. Jessie is 11 years of age. 3. Petitioner has resided at 66 Old Stonehouse Road, Carlisle, PA since 'i September 24, 2001. 4. Hurley, who died on August 26, 1993. 6. Jesse, who was almost three (3) years old at the time of his father's death, has been in the legal custody of Petitioner since his birth. 7. Jesse has resided with his maternal grandparents, Ann and Clyde Kramer continuously since June, 1995. 8. Petitioner, on Jesse's behalf, requests the addition of Hurley, Jesse's natural father's surname, changing his current surname from Domene to Domene-Hurley. 9. Petitioner believes and, therefore avers, that the change of her son's surname from Domene to Domene-Hurley is entirely consistent with Jesse's wishes and will be in Jesse's best interests. 10. Jesse, for the past year, has expressed his strong and unwavering desire to assume his father's surname. 11. J esse and Petitioner believe that the addition of his father's surname will provide him with a better sense of connection with his deceased father. 12. J esse receives Social Security benefits as a result of his father's death. 13.
