PETITION FOR NAME CHANGE
PURSUANT TO 54 Pa. C. S. !l701
The Petitioner, Rena M. Bowen, by her attorneys, the Family Law Clinic, hereby
petitions this Honorable Court to enter a decree changing her minor son's name from Shawn
Matthew Jean Bowen to Shawn Matthew Jean Karns, and respectfully represents that:
1. The Petitioner is Rena M. Bowen, who lives at 132 South Bedford Street, Carlisle,
Cumberland County, Pennsylvania.
2. Petitioner is the biological mother of Shawn Matthew Jean Bowen (hereafter
"Shawn"), who was born on October 20, 1998, in Chambersburg, Franklin County,
Pennsylvania. A true copy of Shawn's birth certificate is attached as Exhibit "A" and is
incorporated herein by reference,
3. Shawn's social security number is 179-78-2255.
4. Shawn's father is John C. Bowen ("father"), whose last known address is P.O, Box

188, 13500 Fleetwood Drive, Nokesville, Virginia 20182-0188.
5. Rena Bowen and John C. Bowen were married on November 29, 1997.

6. Rena Bowen and John C. Bowen were separated on July 15, 1998, six months prior
to Shawn's birth.
7. Rena Bowen and John C. Bowen were divorced on November I, 2000.

8. Petitioner avers that it is in the best interest of Shawn Matthew Jean Bowen to have
his name changed to Shawn Matthew Jean Karns, because:
a) Mother has been divorced from the father and plans to retake her maiden name of
Rena M. Karns, contingent upon her son's name being changed to Shawn Matthew Jean
Karns,
b) Father has had no contact with Shawn since his birth in October 1998.
c) Father did not show up the one time arrangements were made for him to see his son,
on Sunday, December 28, 1998 at 2:00 p.m..
d) Father owes child support for Shawn, but is in arrears in the amount of $8970.41.
e) Petitioner is estranged from the Bowen family, and Shawn has had no contact with
the Bowen family.
9. Since the child's birth, Petitioner and Shawn have lived at their current address,
132 South Bedford Street, Carlisle, Cumberland County, Pennsylvania, and Father is aware of
this address.
10. There are no judgments, liens, decrees or the like against Shawn Matthew Jean
Bowen or Shawn Matthew Jean Karns.
WHEREFORE Petitioner prays that the Court:
a) Set a time and place for a hearing on the above-captioned matter;
b) Direct Petitioner to serve this Petition and the attached Order on John C.
Bowen, the biological father of the child, via United States mail, certified, restricted delivery,
return receipt requested, postage prepaid and by regular first class mail;

Shawn is almost 3 ears old. He's a loving child who wants to know whose his dadd .
at do I tell him? Certainly, not the truth. I tell him your at your house, He tried to run
down stairs to see you. It broke my heart. I said no he lives somewhere else and you
were there. I thought you should know.
John, I would never deny Shawn a father. He deserves one. My son is everything to me,
He is so smart, He loves music, books and cars, He is so big for his age. His teacher and
friends are always commenting on how intelligent he is. HE talks so well. His manners
are impeccable. He is my precious child, Being a parent is amazing. Your choosin!;l to
miss out on so much. Maybe its easy beCaUse you never saw him. But if you just looked
into though eyes just once, ou couldn't walk awa .

child. I want him to know that you couldn't be there, But, one day he will put it together.

gave him her middle name. Shawn Matthew Jean. My grandfather died a year and a half
later of cancer as well. He asked me to give him our family name. I want to do this. I
want to give him this. Shawn was my grandfather's pride and joy. Which is probably no

SPECIAL ORDER DIRECTING METHOD OF SERVICE
Petitioner, RenaM. Bowen, by her attorneys, the Family Law Clinic, respectfully
requests this Court to enter an Order directing method of service on John C. Bowen,
biological father of Shawn Matthew Jean Bowen, by: I) certified and regular mail to Mr.
Bowen's parents' address; and 2) by publication, once in the Cumberland County Law Journal
and once in the Carlisle Sentinel, upon consideration of the following:
I, The Petitioner, Rena M. Bowen ("Mother"), is the plaintiff and biological mother of
Shawn Matthew Jean Bowen.
2, The Respondent, John C. Bowen ("Father"), is the defendant in the above-captioned
matter and the biological father of Shawn Matthew Jean Bowen,
3, On November 2,2001, Mother filed a Petition to change Shawn Matthew Jean
Bowen's name to Sean Matthew Jean Karns.
4, Despite numerous attempts since November 19, 2001, Mother has not successfully
served Father with the Petition for Name Change.
5. Father has not had any contact with his child since December 1998.
6. Since the child's birth, Mother and child have lived at their current address, 132 South
Bedford Street, Carlisle, Cumberland County, Pennsylvania. Father is aware of this

7. Mother has engaged in numerous good faith efforts to locate Father, including the
following:
a. Beginning in March of 1999, the Family Law Clinic represented Rena
Bowen in a divorce action against John C. Bowen
b. After a number of unsuccessful attempts to locate Mr. Bowen to serve the
divorce complaint, the Family Law Clinic served Mr. Bowen on February
18, 2000 by mailing the complaint via United States mail, certified,
restricted delivery, retum receipt requested, and regular first class mail to
his parents' address at 2212 Youngs Drive, Haymarket, Virginia, 20169,
Mr. Bowen signed for the certified mail at that address,
c. In August of 2000, the Family Law Clinic was informed that Mr. Bowen
was no longer living with his parents. The Family Law Clinic served the
remaining documents relating to the divorce, on Mr. Bowen at 213 7
Bentwood Circle, Apartment lA, Columbus, Ohio, 43235, This address
was provided by the Domestic Relations Office.
d. On April 24, 2001, the Family Law Clinic sent a letter by certified mail to
John C. Bowen at the above-listed address, regarding Rena Bowen's
action for name change of her son,
e, On May 18,2001, the letter was returned to the Family Law Clinic
because the mail had remained unclaimed, and the forwarding address was
unknown,

f On September 17,2001, the Family Law Clinic contacted Mr, Bowen's
support officer, Frank Goshorn, at the Domestic Relations Office, to
inquire about Mr. Bowen's address, Mr. Goshorn informed the Family
Law Clinic that Mr, Bowen could no longer be reached at his Columbus,
Ohio address, Mr, Goshorn informed the Family Law Clinic that he had
contacted Mr. Bowen's mother, but Mr, Bowen's mother said that she had
no information regarding her son's location,
g, On September 17, 200 I, Mr. Goshorn wrote a letter to the Family Law
Clinic to inform the Family Law Clinic that the Domestic Relations Office
was making efforts to locate John C, Bowen via data entry in the National
and State new hire system, subpoena to the D.S, Department of Education,
and parent locators through CSNet.
h, On November 19, 2001, the Family Law Clinic received another address
from the Domestic Relations Office: P,O, Box 188, 13500 Fleetwood
Drive, Nokesville, Virginia, 20182-0188. The Clinic attempted to serve
the Petition for Name Change on John C. Bowen at this address by
certified and regular first class maiL
L On November 26,2001, the Petition was returned to the Family Law
Clinic because the mail had remained unclaimed, and the forwarding
address was unknown,
j. On December 6,2001, the Domestic Relations Office provided the Family
Law Clinic with another address: 610 Acacia Drive, Marysville, Ohio,

Name Change on John C. Bowen at this address by certified and frrst class

k On January 4, 2002, the Petition was returned to the Family Law Clinic
because the mail had remained unclaimed, and the forwarding address was
unknown,
L At the end of January, 2002, The Family Law Clinic again contacted Mr,
Goshorn at the Domestic Relations Office to request information regarding
Mr, Bowen's location, Mr, Goshorn informed the Family Law Clinic that
the Domestic Relations Office has been unsuccessful in its numerous
attempts to locate John C, Bowen,
m. The Family Law Clinic believes that John C. Bowen's father, Clifford
Bowen, lived at 2212 Youngs Drive, Haymarket, Virginia 20169 as of
February, 2000. The Family Law Clinic does not know whether any
Bowen family member continues to reside at that address. The Family Law
Clinic obtained the following telephone number for Clifford Bowen at that
address via Internet telephone directories: (703) 754-9899.
n. During the month of February, 2002, the Family Law Clinic called (703)
754-9899, the telephone number presumed to be Clifford Bowen's. After
numerous attempts, the Family Law Clinic finally reached someone at
(703) 754-9899, but it was not Clifford Bowen. The person at that
number was J. Stuart Painter of 6780 Hollow Glen Court, Gainesville, VA.

o. The Family Law Clinic was informed that Clifford Bowen had that number
disconnected two years ago, and the number now belongs to Mr. Painter at
a different address. The Family Law Clinic was not able to obtain an
updated telephone listing for Clifford Bowen in Haymarket, Virginia.
8. In spite of all of these efforts, Mother does not know where Father is and has no way
of obtaining such information.
9, Father has not entered an appearance in this case, either personally or through counsel.
WHEREFORE, Mother requests that this Court enter an order directing method of service the
Petition for Name Change as follows: by mailing Petition and Notice to Mr, Bowen's parents'
address at 2212 Youngs Drive, Haymarket, VA 20169 by certified and regular mail, and by
publishing the Notice of Hearing attached hereto as Exhibit A, once in the Cumberland County
Law Journal and once in the Carlisle Sentinel.

Clinic, located at 45 North Pitt Street, Carlisle, Cumberland County,

2, Beginning in March of 1999, the Family Law Clinic represented Rena Bowen in a
divorce action against John C. Bowen.
3. After a number of unsuccessful attempts to locate Mr. Bowen to serve the divorce
complaint, the Family Law Clinic served Mr. Bowen on February 18, 2000 by

Virginia, 20182-0188. The Clinic attempted to serve the Petition for Name
Change on John C. Bowen at this address by certified and regular first class mail.
10. On November 26, 2001, the Petition was returned to the Family Law Clinic
because the mail had remained unclaimed, and the forwarding address was
unknown.
11. On December 6,2001, the Domestic Relations Office provided the Family Law
Clinic with another address: 610 Acacia Drive, Marysville, Ohio, 43040-8669.

12. On January 4, 2002, the Petition was returned to the Family Law Clinic because
the mail had remained unclaimed, and the forwarding address was unknown.
13. At the end of January, 2002, The Family Law Clinic again contacted Mr. Goshorn
at the Domestic Relations Office to request information regarding Mr. Bowen's
location. Mr. Goshorn informed the Family Law Clinic that the Domestic
Relations Office has been unsuccessful in its numerous attempts to locate John C.
Bowen.
14. The Family Law Clinic believes that John C. Bowen's father, Clifford Bowen, lived
at 2212 Youngs Drive, Haymarket, Virginia 20169 as of February, 2000. The
Family Law Clinic does not know whether any Bowen family member continues to
reside at that address. The Family Law Clinic obtained the following telephone
number for Clifford Bowen at that address via Internet telephone directories:
(703) 754-9899.
15. During the month of February, 2002, the Family Law Clinic called (703) 754-
9899, the telephone number presumed to be Clifford Bowen's. After numerous
attempts, the Family Law Clinic finally reached someone at (703) 754-9899, but it
was not Clifford Bowen.
16, The Family Law Clinic was informed that Clifford Bowen had that number
disconnected two years ago, and the number now belongs to J. Stuart Painter,
6780 Hollow Glen Court, Gainesville, VA. The Family Law Clinic was not able to
obtain an updated telephone listing for Clifford Bowen in Haymarket, Virginia,

contacted Mr, Bowen's parents in an effort to fmd him,

the National and State new hire system, subpoena to the U.S. Department of
Education, and parent locators through CSNet. The Domestic Relations Office
has been unable to locate Mr, Bowen,
7, Because the Family Law Clinic has been unable to locate Mr, Bowen to serve him
personally, Petitioner intends to file a Petition for Service by Publication,
8, Petitioner asks that this Court reschedule the hearing in this matter in order to
allow additional time to file the Petition for Service by Publication and serve the
Petition for Name Change on John C. Bowen in a manner acceptable to the Court,
