certain tract of land including improvements thereon, owned by her and described

For Sewer services rendered from 03/0~./0! to 04/0~./03. Said services were rendered
pursuant to ordinance. Attached hereto is a statement of the kind and character of the
services so rendered and the price charged therefor, said statement being a true copy of
the owner's account with Hampden Township which sum of $854.19 was duly assessed
against the hereinabove-named owner and the premises on 03/01/0! and for which sum, with
interest, penalties and costs, a lien is claimed against the above-described premises Jn
accordance with the Act of May 16, !g23, P.L. 207 (53 P.5. Section 7101, et seq.), as
amended and supplemented, and the Act of May 2, ~945, P.L. 382 (53 P.5. Section 301,
et sex/), as amended and supplemented.
