I, Anthony T. McBeth, Esquire, Attorney for Petitioner in the captioned action, hereby swear
that I have provided notification of the proceeding, to the Petitioner's father, as required by this
Court's Order dated February 4, 2002. I have provided a copy of the name change petition, a copy
of the notice that we are arranging to have published in the Cumberland County Legal Journal and
in the Carlisle Sentinel, and a cover letter. Copies of the notice and my cover letter are attached
hereto, marked Exhibits "A" and "B" respectively, and incorporated herein by reference. We have
sent the notice package, on the date entered below, by certified mail and by first class mail. Copies
of the two receipts evidencing that fact are attached hereto, collectively marked Exhibit "C" and
incorporated herein by reference, to the following person at the last known address that we have for

PETITION OF BRENT EUGENE ASH FOR THE CHANGE OF IDS NAME
TO BRENT TALA ROTHROCK
1. Petitioner Brent Eugene Ash is a resident of Enola, Cumberland County, PeIn1Sylvania and
has resided at the following address for more than the last five years: 5404 Legen Lane, Enola,
PeIn1Sylvania 17025.
2. The Petitioner was born on August 15, 1978 in Harrisburg, Dauphin County, Pennsylvania.
His parents are LeRoy Eugene Ash (father) and Wanda Ann Ash (nee: Rothrock) (mother).
3. Petitioner's mother is a member ofhis household, and Petitioner's father lives at 746 Erford
Road, Camp Hill, Cumberland County, PeIn1Sylvania.
4. Petitioner would like to change his name from Brent Eugene Ash to Brent Tala Rothrock for
the following reasons:
a.) Petitioner would like to change his surname from Ash to Rothrock because
Rothrock is his mother's maiden name, and Petitioner wishes by that change to reflect the fact that
his mother has raised and nurtured and assisted him throughout his life, whereas his father has
exercised minimal involvement in Petitioner's life, despite the fact that numerous entreaties have been
made to the father to become more involved in the Petitioner's life;
b.) The Petitioner would like to change his middle name from Eugene to Tala because
he does not want his middle name to be the same'llS his father's middle name, and has chosen Tala
as his new middle name because that name is appealing to the Petitioner.

5 . Your petitioner is not married and has not mthered any children in his lifetime.
6. There are no judgments or decrees of like character of record against the Petitioner.

requested change of name as contained herein, to ultimately enter an Order changing Petitioner's
name from Brent Eugene Ash to Brent Tala Rothrock, and to provide any other relief this Court
