PETITION FOR CHANGE OF NAME
AND NOW, comes Cheyenne Raenelle DeBraganza-Hill in and through her mother
Holly Y. Moore and her step-father Ruben Moore, by and through their attorney, Andrew H.
Shaw, and files this Petition for Change of Name and in support thereof avers as follows:
1. Cheyenne Raenelle DeBraganza-Hill, (hereinafter "Petitioner") was born on
March 14, 1996 and is currently 11 years old.
2. Petitioner resides at 407 Scott Drive, Shippensburg, Cumberland County,
Pennsylvania with her birth mother, Holly Y. Moore, her mother's husband,
Ruben Moore and Ruben Moore's son, Ruben Moore III.
3. Petitioner's biological father is William Hill, Jr., who resides at 8017 Walker
Street, Philadelphia, Philadelphia County, Pennsylvania.
4. Petitioner seeks to have her name changed from Cheyenne Raenelle DeBraganza-
Hill to Cheyenne Raenelle Moore.
5. Petitioner seeks a change of name because she desires to share the same last name
as her birth mother, birth mother's husband and her birth mother's husband's son.

6. Petitioner's biological father, William Hill, Jr., maintains minimal contact with
Petitioner, does not provide any financial support to Petitioner, and is not
involved in Petitioner's life.
7. Petitioner's residence for the past 5 years has been as follows:
a. 407 Scott Drive, Shippensburg, Cumberland County, Pennsylvania (April 1,
2006 to present);
b. 1740 Wisteria Drive, Chambersburg, Franklin County, Pennsylvania
(December 7, 2005 to March 31, 2006);
c. 2425 Fitler's Walk, Philadelphia, Philadelphia County, Pennsylvania (April
2005 to December 6, 2005);
d. 2416 Fitler's Walk, Philadelphia, Philadelphia County, Pennsylvania (March
14, 1996 to Apri12005).
WHEREFORE, Petitioner respectfully requests this Honorable Court to grant her request
to change her name from Cheyenne Raenelle DeBraganza-Hill to Cheyenne Raenelle Moore.
