Swope, who respectfully states as follows: 1. The Petitioner is SEAN SAMUEL ALBERT, an adult individual, whose date ofbirth is May 3, 1978. He was born in Washington, D. C. 2. The Petitioner currently resides at and has resided at 1821 Creek View Court, New Cumberland, Cumberland County, Pennsylvania 17070 since January 15, 2006. 3. Prior to January 15, 2006, Petitioner resided at the following addresses: - From August 15, 2003 to January 15, 2006 at 320 Railroad Street, 2' Floor, Palmyra, Lebanon County, Pennsylvania 17078; - From June 1, 2003 to August 15, 2003, at 122 South 290' Street, Apt. 1, Harrisburg, Dauphin County, Pennsylvania 17103; - From 2002 to June 1, 2003 at 200 Hamilton Street, Harrisburg, Dauphin County, Pennsylvania 17102. - From September 1, 1999 to 2001 at Shippensburg University, Lackhove Hall and Seavers Residence Hall, 1871 Old Main Drive, Shippensburg, Cumberland County, Pennsylvania 17257; - From May 3, 1978 to September 1, 1999 at 5025 Sunset Drive, Harrisburg, Dauphin County, Pennsylvania 17112. 4. Petitioner desires to change his name to that of his partner. 5. There are no judgments or decrees of like character in this or any other jurisdiction against your Petitioner. WHEREFORE, Petitioner requests the Court pursuant to Act of December 16, 1982, P.L. 1309, No. 295, Section 2, 54 Pa.C.S.A. Section 701, et seq., to: A. Fix a date for hearing on this Petition; B.
