PETITION TO CHANGE NAME
AND NOW comes Tham Hong Nguyen by and through her attorneys Christopher
S. Lucas & Associates and relates the following facts:
1. Tham Hong Nguyen is an adult individual, bom in 1980 in the People's Republic
of Viet Nam to Le Thi Nguyen (mother) and Thanh Van Nguyen (father).
2. In 1982, Tham Hong Nguyen emigrated to the United States.
3. In 1988, Tham Hong Nguyen was naturalized as a United States citizen.
4. Tham Hong Nguyen is a resident of Cumberland County Pennsylvania, residing at
5245 Joshua Rd. Mechanicsburg, PA 17055.
5. Tham Hong Nguyen wishes, for reasons of phonetic convenience and in order to
facilitate business and personal communications, to change her given name to
"Malina".
WHEREFORE, Tham Hong Nguyen respectfully request that this court issue an
order changing her name from Tham Hong Nguyen to Malina Nguyen.

I, Tham Hong Nguyen verify that the facts set forth in the foregoing Petition To
Change Name are true and correct to the best of my knowledge, information and belief
and are consistent with testimony I would provide if called to testify under oath in the
captioned proceeding. This Verification is made subject to the penalties prescribed under
18 Pa.C.S. § 4904 relating to unsw, om falsification to authorities.
