Thomas D. Gould, and requests P.S. Section 701, to issue biological son, SEAN MICHEAL MILLER, on the following: 1. Kristina A. Burns, BURNS, by and through her attorney, this Honorable Court, pursuant to 54 a Decree changing the name of her to SEAN MICHAEL Bb'RNS, based hereinafter referred to as Mother, is an adult citizen who resides at 3-S Spruce Lane, Carlisle, Cumberland County, Pennsylvania 17013. 2. Mother is the biological mother of Sean Micheal Miller, born April 8, 1992, (although the birth certificate states April 9, 1992), hereinafter referred to as Child. 3. No father is listed on the birth certificate. 4. Mother gave the Child the surname of her Septfather. 5. In 1991 Mother had a brief relationship Galbraith(sp), who was an over-the-road truck driver. 6. 7. with Robert Robert Galbraith is the biological father of the Child. Robert Galbraith has never held himself out to be the father of the Child. 8. Robert Galbraith, upon learning that Mother was pregnant left the area and has never had contact with Mother or Child. 9. Mother last had contact with Robert Galbraith in late 1991 or early 1992. 10. Mother has no information as to the current location of Robert Galbraith. 11. Greg A. Burns was present at the Child's birth and has held the Child out as his own since birth. 12. Mother married Greg A. Burns on September 11, 1996. 13. The best interests of Child will be served by changing her last name to Mother's because: a. Child lives in Mother's household. b. Ail the members of Mother's household have the last name of Burns. c. Mother's name is well respected in the community. 13. Child has no judgments or decrees of record or any other matter pending of like character against him in this or any other jurisdiction. WHEREFORE, Kristina A. Burns respectfully requests this Honorable Court to set this matter for hearing and enter an Order directing Petitioner to give Legal Notice as required by statute to all other interested parties of the filing of this action and the date and time set for Hearing, which shall not be less than one month or more than three months after the filing of this Petition. After the Hearing it is respectfully requested that this Honorable Court issue a Decree changing the name of Sean Micheal Miller to Sean Michael Burns. Respectfully submitted, Thomas D. Proper certification from the Prothonotary's Office verifying that there are no judgments, decrees of record or any other like character against the petitioner shall be submitted at the hearing. It is further ordered that a notice of said hearing shall be made by one (1) publication in both the Cumberland Law Journal and in a newspaper of general circulation in Cumberland County, Pennsylvania.
