AND NOW, this .day of ,2003, Petitioner, Davin Charles Emlet by and
through his attorneys, MARTSON DEARDORFF WILLIAMS & OTTO, files the following Petition
for Change of Name:
1. Petitioner, Davin Charles Emlet, is a single adult individual residing at 30 East Slate
Hill Road, Carlisle, Cumberland County, Pennsylvania. Petitioner has resided at this address since
September 2002.
2. Immediately prior to his current address, Petitioner resided at 2086 Newville Road,
Carlisle, Cumberland County, Pennsylvania from1994 to September 2002.
3. Petitioner desires to have his name changed from Davin Charles Emlet to DAV1N
CHARLES WAUGAMAN.
4. Petitioner desires to have his name changed because of a personal choice to take his
mother's maiden name.
5. Petitioner is a citizen of the United States of America, having been born in
Harrisburg, Pennsylvania on April 11, 1981.
6. Petitioner is a single adult individual who is the age of twenty-two (22) years.
7. If this Petition is approved, Petitioner intends to use the name DAV1N CHARLES
WAUGAMAN consistently, non-fraudulently, and exclusively.
8. It is not the purpose of this Petition to avoid any taxes or any other debts.
WHEREFORE, Petitioner requests this Honorable Court to enter an Order of Court changing
his name from Davin Charles Emlet to DAVIN CHARLES WAUGAMAN.
