I, Plaintitl: David S, Masland, M,D., isan adult individual rcsidingat205 Heisers Lane,
Carlisle, Cumberland County, Pennsylvania,
2. Defendant, Emmanuel S, Shughart, is an adult individual residing at 531 Pleasant
Hall Road, Carli sic, Cumberland County, Pcnnsylvania.
3, On June 29, 1995, Dcfendant signed a Promissory Note (the "Note") whereby
Defendant unconditionally promised to pay to the order of Plaintiff Fifteen Thousand Dollars
($ I 5,000,00), with interest at the rate of seven percent (7.0%) per annum. A copy of said Note is
allached hereto as Exhibit "A" and is incorporated herein by reference.
4, Defcndant has made no payments, in cash or in kind, under the Note since June 9,
1997,
