Abe entered incorporating the terms of this Stipulation.

5„ The parties will share legal. custody of Katlyn with
the understanding that that provides to each of. them the
right to have a say in important decisions regarding Kallyn's
life, and to obtain information from schools and doctor and
other agencies regarding the child. In the event Katlyn. is
involved in a meaical emergency, the party who has phys~.cal
custody of her will immediately contact the other. party The
parties will keep one another advised of regular docto r
dentist, and/or optometrist appointments in advance of there
taking place. Ir. the event it becomes necessary to charge
Katlyn's physician, Father will advise Mother cf. the nel~i
doctor caring for her.
Furthermore, both parties may have access to all '~
information regarding Katlyn's education. Mother will ~~~
contact Katlyn's school to arrange for duplicates of al~_
notices routinel}' sent to Father to be sent to her as w~ll.

In the event that. the school will not send duplicate '~
information of one kind or another, Mother will. advise Father
and Father will provide copies of such information: to Mather.

6. Father shall have primary physical custody ofl

A. Mother shall have custody of :~atlyn every
other weekend. Custody shall commence at .5:00 p.m. Fric.ay
and conclude at 5:00 p.m. Sunday. Transportation for weekend
custody shall be provided by Mother.
B. Th.e parties will observe the following
schedule for holidays as follows: In 2012, Father will have
Katlyn for Thanksgiving and Christmas Day and Mother will.l
have Katlyn for Easter and Christmas Eve. In 2013, Father
will have Katlyn for Easter and Christmas Eve and Mother, will
have Katlyn for 'T'hanksgiving and Christmas Day. Custody of
the child on those holidays shall extend from 9;00 a.m. until
8:00 p.m.
Father and Mother will communicate and arrang6~
other holidays: New Year's, Memorial Day, Fourth of Ju~_y and
',Labor Day shortly before each upcoming holiday.
Additional, Mother may have custody of the ch~_ld
during the child's Christmas vacation on three other days at
(times to be arranged with Father.
C. Mother shall have custody of the child obi her
birthday and on her son, Patrick Swartz, birthday. Fattier
will have custody of the child on his birthday and on ha~~s
daughter, Hayley Clepper, birthday. If the birthday is on a
weekend, custody will be from 9:00 a.m. until. 5:00 p.m.' If
the birthday fal7_s on a weekday, custody will be from 6..00

p.m. until 9:00 p.m.
D. Mother will have custody for a seven-day'
period in June, July and August. Mother's weeks will irj~clude
her own weekends of partial custody and not Father's weekends
of partial custody.
7. The parties will have free and open communicat#:ion
with the child by telephone or by e-mail.
8. If either party intends to remove Katlyn fromlthe
Commonwealth, he or she will notify the other cf the
destination, including address and telephone number, anc~ the
approximate length of the stay. Such notice should be given
ordinarily at least two weeks prior to the trip. In th~ case
of an emergency, such as the death of an out-of-state
relative, the party wishing to remove the child from th$

provide the address and telephone number where the chip may
be reached.
9. During the time when the child is with. them, ~:he
parties shall personally supervise the child. In the
alternative, the child may be temporarily supervised by'~a
person deemed by the custodial parent to be a responsible

10. The parties intend that the terms of this
Stipulation be entered as an Order of court.
