Frcd D. Carbaugh and Judy B. Carbaugh of 196 Lawrence Lane, Carlisle, Pennsylvania
17013. hereinafter collectively referred to as "DEBTOR", after November 19, 1998, does promise
to pay to the order of Lynford K. Donivan, Ten Thousand Sixty-One and 00/100
($10,061.00) without defalcation, value received, with interest from July 22, 1996 althe rate of
twelve and 3/4 (12.75%) per cent, per annum, payable intercst only in monthly
insta11men~n the outstanding principal balance, the entire prineipal balance being due on and after
July 22, 2001, And further DEBTOR does hereby authorize and empower any Attorney of any
Court of Records of Pennsylvania, or elsewhere, to appear for and to enter Judgment against
DEBTOR for the above sum, with or without declaration, with costs of suit, release of errors,
without stay of execution, and with five per cent, added for collecting fees; and DEBTOR also
waives the right of inquisition on any real estate that may be levied upon to collect this note, and
does hereby voluntarily condemn the same and authorize the Prothonotary to enter upon the FI.
FA. DEBTOR's said voluntary condemnation, and DEBTOR further agrees said estate may be sold
on a FI. FA., and does hereby waive and release all relief from any and all appraisement, stay or
exemption laws of any State, now in force, or hereafter to be passed. DEBTOR hereby certifies
that DEBTOR'S annual income is in excess $10,000.00 and fully understands thc terms and
conditions of this Note.
I hereby certify that the precise residence of the Judgment Creditor in this Judgment is 116
West Hillcrest Drive, Carlisle, PA 17013.
