4. The labor and materials for which the debt is due
consisted of providing and installing various building
materials for the rehabilitation and remodeling of an existing
home,
5, The labor and materials furnished consisted of the
following:
(a) Labor: 1,534 hours of work at the standard
rate of Twenty Five and 00/100 Dollars ($25,00) per hour, for
a total of Thirty Eight Thousand Three Hundred Fifty and
00/100 Dollars ($38,350,00), in connection with demolition
work, repairing floors, installation of insulation and
drywall, installation of windows, treating fire damaged wood
with bin paint, cleaning stove, framing of basement,
installation of exterior doors, installation of shelving, trim
work, administrative work, general labor including, but not
limited to, screwing floors, caulking, doorknobs, cleaning,
and scraping windows, An itemized list of the hours worked is
attached hereto, made part hereof, is incorporated herein by
reference, and is marked Exhibit IP-2",

(b) Materials: Building permit, Waste Management,
dumping fee, Eberly's Lumber, Lowe 's, The Home Depot, B&C
Fasteners, Carter Lumber, The Lumber Yard, various building
suppliers, gasoline and kerosene, CER Rentals for use of saw,
shelving, Norandex windows, Kohl Building Products, Systems
Distribution, various building suppliers and John R, Walker
Heating & Plumbing, Lon Smith, The Paintsmith, and Matt
Kostelac for installation of flooring for a total of Fifty
Nine Thousand Two Hundred Thirty six and 82/100 Dollars
($59,236,82), An itemized list of the material suppliers is
attached hereto, made part hereof, is incorporated herein by
reference, and is marked Exhibit IP-3",
6, The Claimant completed its work at the property on
December 22, 1998, which is less than four (4) months before
the filing of this Claim,
7 , Claimant has been paid the sum of seventy Nine
Thousand and 00/100 Dollars ($79,000,00), and there is due and
owing a balance of Eighteen Thousand Five Hundred Eighty six
and 82/100 Dollars ($18,586,82) for which claim is made of
Eighteen Thousand Five Hundred Eighty six and 82/100 Dollars
($18,586,82). An itemized list of the payments is attached
