STIPULATION FOR CHILD CUSTODY
AND NOW, this ____ day of
, 1999,
the parties hereto, Bach T. Le and Tham H. Duong, do hereby
stipulate and otherwise agree as follows regarding the care and
custOdy of their minor child:
1. Plaintiff is Bach T. Le, an adult individual
residing at 1023 Drexel Hill Boulevard, New Cumberland,
Cumberland County, Pennsylvania 17070.
2. Defendant is Tham H. Duong, an adult individual
residing at 5 Birchland Lane, Apt. 107, Camp Hill, Cumberland
Co~ntYI Pennsylvania 17011.
3. The subject minor child is Tai T. Duong, born
September 23, 1987.
4. The parties shall have shared legal custody of the
subject minor child. They shall consult with each other relative
to all important decisions concerning the subject minor child,
inclUding such matters as health, education, and religion.

5. It is agreed between the parties that Plaintiff,
Bach T. Le, shall havo primary physical custody of the subject
minor child.
6. It is agreed between the parties that Defendant,
Tham H. Duong, shall be entitled to partial periods of physical
custody, pursuant to the following schedule:
a. Father shall be entitled to two days of
partial custOdy per week. It is acknowledged that
Father works an irregular schedule, and it is therefore
further agreed that the two days in question shall
occur in conjunction with his days off from work, and
the exact details shall be agreed upon between the
parties. Father shall give Mother reasonable advance
notice relative to the times in which he wishes to
exercise his periods of partial custody.
b. Father shall be entitled to two weeks each
summer. They may, at his option, be consecutive or
non-consecutive. Further, this shall be subject to 30
days notice to Mother.
c. The parties shall share holidays in as equal a
manner as possible.
d. Father shall be entitled to additional periods
of partial custody, with the exact details to be
subject to the agreement and express consent of
Plaintiff, Bach T. Le.
7. During any period of custody or visitation the
parties to this Order shall not possess or use any controlled
substance, neither shall they consume alcoholic beverages to the
point of intoxication. The parties shall likewise assure, to the
extent possible, that other household members and/or house guests
comply "ith this prohibition.
