present at the Conference at the request of either party. Failure
to appear at the Conference may provide grounds for entry of a
temporary or permanent order.

6. The minor children have resided at the following
addresses, in the custody of the following individuals:
a) From December, 1996 to present - the subject
children reside with Plaintiff, Plaintiff's wife, Lisa R. Cooley,
and her son,, Michael R. Allen, at 613 B Geneva Drive, Apt. 13,
Mechanicsburg, Cumberland County, Pennsylvania.
b) From December, 1995 until December 1996 - the
subject children resided with Plaintiff at 1809 North Second
Street, Harrisburg, Dauphin County, Pennsylvania.
c) From 1992 until December, 1995 - the subject
children resided with Defendant at several addresses in Dauphin
County and Lancaster County.
d) From birth until June, 1992, the subject minor
children resided with Plaintiff and Defendant in Harrisburg,
Dauphin County, Pennsylvania.
7. There have been no prior actions for custody of the
subject minor children in this or any other jurisdiction.
8. The Plaintiff is not aware of the existence of any
other individuals who have any type of claim whatsoever regarding
the custody of the subject minor children.
9. The Plaintiff believes, and therefore avers that
he is much better able to meet the needs of the subject minor
children than the Defendant.
10. The Plaintiff believes, and therefore avers that it
is in the best interest of the subject minor children that they be
placed in his legal and physical custody.
WHEREFORE, Plaintiff requests this Honorable Court award
him custody of the subject minor children.

ORDER OF COURT
AND NOW, this - \ day of 1999, upon
consideration of the attached Custody Conciliation Report, it is ordered
and directed as follows:
1. The Father, Wayne V. Cooley, and the Mother, Minerva Cooley,
shall have shared legal custody of Wayne V. Cooley, Jr., born September 16,
1989, and Tylon D. Cooley, born October 6, 1991. Each parent shall have an
equal right, to be exercised jointly with the other parent, to make all
major non-emergency decisions affecting the Children's general well-being
including, but not limited to, all decisions regarding their health,
education and religion.
2. The parties shall share having physical custody of the Children in
accordance with the following schedule:
A. The Mother shall be entitled to have custody of the Children
on alternating weekends from Friday through Sunday, for one
weekday evening per week and for up to 3 nonconsecutive weeks
during the summer school vacation. The Mother shall provide at
least thirty (30) days advance notice of the dates she selects
for extended periods of summer custody under this provision.
B. The Father shall have custody of the Children at all times not
otherwise specified for the Mother under this provision.
3. The parties shall share having custody of the Children over
holidays as arranged by agreement.
4. In the event neither party is available to take the Children to
their medical appointments, the parties agree that the Father's wife, Lisa
Cooley, shall be authorized to do so.
5. Neither party shall do or say anything which may estrange the
Children from the other parent, injure the opinion of the Children as to

the other parent, or hamper the free and natural development of the
Children's love and respect for the other parent.
6. This Order is entered pursuant to an agreement of the parties at a
Custody Conciliation Conference. The parties may modify the provisions of
this Order by mutual consent. In the absence of mutual consent, the terms
of this Order shall control.
