named individual, who has applied for a name change, has been searched in the Pennsylvania
State Police, Records and Identification Division Central Repository, and;

NAME CHANGE ACT, THE COURT SHALL NOTIFY THE PENNSYLVANIA STATE POLICE
WHEN A NAME CHANGE FOR A PERSON CONVICTED OF A FELONY HAS BEEN
ORDERED. The Pennsylvania State Police, upon receipt of this notice, shall include the change
of name information in the Central Repository as provided for in 18 PA C. S., Chapter 91 (relating

AMENDMENT TO PETITION TO CHANGE NAME
TO THE HONORABLE, THE JUDGES OF SAID COURT:
AND NOW, comes the Petitioner, Paul Donald Sitko, by and through his attorney, Karl
E. Rominger, Esquire of the Law Offices of Paul Bradford Orr, and respectfully represents as
follows:
I. That your Petitioner previously requested this Court to make a name change to the
above-referenced docket.
2. Petitioner served a copy of the same upon the Pennsylvania State Police along
with a fingerprint card
3. Petitioner has since decided to add his grandfather's name to his new name.
4. Petitioner has decided that his grandfather participated heavily in his development
and wishes to honor him also.
5. The previous Petition is incorporated herein by reference with the exception of the
following change.
6. Petitioner Paul Donald Sitko petitions this Court to change his name to Paul
Donald Murphy-Ahles.
7. Again, Petitioner has no fraudulent intent in changing his surname, and in fact is
doing so out of his natural love and affection for those who raised him and to
honor them.
WHEREFORE, Petitioner respectfully requests that this Honorable Court grant his name
change and schedule a hearing in this matter.

AND NOW, comes the Petitioner, Paul Donald Sitko, by and through his attorney, Karl

That your Petitioner is Paul Donald Sitko, an adult individual, who resides at 3514
Beech Run Lane, Mechanicsburg, Pennsylvania 17055.
2. That your Petitioner is twenty-one (21) years of age, having been born on May 1,
1978 at Camp Hill, Pennsylvania.
3. That your Petitioner has been a resident of Pennsylvania since birth.
4. That your Petitioner has never been married and has no children.
5. That Your Petitioner desires to change his name for more appropriate indication of
family background by substituting his stepfather's name of Ahles and thus changing his name
from Paul Donald Sitko to Paul Donald Ahles.
6. That Your Petitioner has no fraudulent purpose in changing his surname, having
no creditors, nor judgments, decrees or other process outstanding against him.
WHEREFORE, pursuant to 42 Pa.C.S. Section 702, your Petitioner respectfully requests
our Honorable Court to Order his name of Paul Donald Sitko be changed to a new full name of
Paul Donald Ahles.

ORDER
AND NOW, December 3, 1999, on hearing in the above-captioned petition for change of
name, the Court finds as follows:
1. Paul Donald Sitko filed with this Court a Petition to Change Name on June 30,
1999, and an Amended Petition to Change Name on September 10, 1999, seeking
to change his name from "Paul Donald Sitko" to "Paul Donald Murphy-Ahles"
pursuant to 54 PA.C.S. §701 et seq.
2. The Court entered an Order on October 11, 1999, fixing a hearing on said Petition
for December 3, 1999, and directed that notice of said hearing be published one
time in the Cumberland Law Journal and a newspaper of general circulation.
3. The required notices were published as follows:
a. Cumberland Law Journal - October 15, 1999
b. The Sentinel - October 12, 1999
4. The Petitioner is a resident of Cumberland County.
5. The Petitioner is not married and has no children.
6. No objections to the Petition have been filed and none were received at the
hearing.
The Petitioner appears to have no fraudulent purpose in changing his name.
THEREFORE, it is ordered and directed that Petitioner is permitted to change his name
from "Paul Donald Sitko" to "Paul Donald Murphy-Ahles," and that Petitioner shall hereafter be
known as Paul Donald Murphy-Ahles.
