Claimant Lester Poling, t/d/b/a/ Lester's Tub Repair, through the undersigned counsel, files
this claim against the improvements and property at 300 Baltimore Road, Shippensburg,
Pennsylvania, Cumberland County, for the payment of a debt due to claimant as a contractor for
labor and materials furnished by claimant in the repair of the improvements. In support of the claim,
the claimant makes the following statement:
1. The owner of the Property is Senior Cottages of America, Inc., t/a Cottages of
Shippensburg, LTD, with an address at 300 Baltimore Road, Shippensburg, Cumberland County,
Pennsylvania.
2. The improvements and the property which are subject to this claim are two story apartment
units, with appurtenant land at 300 Baltimore Road, Shippensburg, Pennsylvania.
3. The labor and materials for which the debt is due were furnished pursuant to an oral
agreement with contractor under which Claimant was to be paid for time and materials, furnished
by it at its standard rates.
4. The labor and materials furnished consisted of the following:
Labor and materials to repair bathroom tubs and kitchen cabinets.
5. The claimant completed his work at the property on March 3, 1999, which is less than
four months before the filing of this claim.
6. Claimant has been paid nothing, and there is due and owing a balance of $ 2,555.00 for
which claim is made of $ 2,555.00.
