' Plaintiffs praecipe sets forth that an affidavit of service of the complaint
was filed on June 21, 1999. While plaintiff filed an Affidavit of Service, which
affidavit sets forth that the complaint was served by certified mail, "[a]s evidenced
by the attached Post Office receipt card dated June 21, 1999," the receipt card is
not attached to the affidavit or has it otherwise been filed of record. We will not
enter a divorce decree where the record does not contain proof of the service of
the complaint.

1. Plaintiff is Brett D. U rich, an adult individual, currently residing at 618 High Street, Ene1_a, Cumberland
County, Pennsylvania, 17025-3161.
2. Defendant is .'Angela R. Urich, an adult individual, currently residing at 540, North Front Street, Rear 2nd
Floor, Dauphin County, Pennsylvania, 17113.
3. Plaintiff and Defendant are buuafide residents of the Commonwealth of Pennsylvania and have been so far at
least sir months immediately previous to the filing of this complaint.
4. Plaintiff and Defendant were married on February 16, 1994 in Hagerstown, Maryland.
5. There have been no prior actions for divorce or annulment between the parties.
6. The Plaintiff has been advised of the availability of marriage counseling and the Plaintiff may have the right
to request that the court require the parties to participate in counseling. Having been so advised Plaintiff
does not desire the Court to order counseling.
7. The marriage is irretrievably broken.
WHEREFORE, Plaintiff prays Your Honorable Court enter a decree in divorce.

2. The marriage of Plaintiff and Defendant is irretrievably broken and ninety days have elapsed from the
date of the filing of the Complaint.
I consent to the entry of a final decree of divorce without notice.
4. I understand that I may lose rights concerning alimony, division of property, lawyer's fees or expenses if I
do not claim them before a divorce is granted.
5. I understand that I will not be divorced until a divorce decree is entered by the Court and that a copy of
the decrees will be sent to me immediately after it is filed with the Prothonotary.
