STATEMENT OF MECHANIC'S LIEN CLAIM AND NOW comes AmQuip Corporation, by counsel, Bernstein Bernstein Krawec & Wymard, P.C., and files this Statement of Mechanic's Lien Claim as a sub-contractor and material supplier to a contractor, pursuant to the Mechanic's Lien law of 1963, 49 P.S. Section 1101 et. seq., against the premises herein described, any and all buildings erected thereupon, and the curtilage appurtenant thereto, for the payment of $9,710.46, together with interest from April 19, 1999, due it as a sub-contractor and material supplier to the contractor under the contract herein described. The following is a statement of its claim: G:\91050620.G 1. AmQuip Corporation is a corporation organized and existing under the laws of Pennsylvania, with a place of business located at 777 Winks Lane, Bensalem, Bucks county, Pennsylvania 19020. 2. The Owners are Bruce L. Long and Virginia S. Long, adult individuals at 110 Long Road, Newville, Cumberland County, Pennsylvania 17241. 3. The contractor is LeBlanc Communications, whose address and place of business is 571 Industrial Drive, Lewisberry, York County, Pennsylvania 17339. 4. This claim relates to materials and labor provided by claimant as a sub- contractor and material supplier to the contractor upon the premises designated at 110 Long Road, Newville, Cumberland County, Pennsylvania, situate in North Newton Township, being more fully described in the Deed conveying said premises to owner, recorded in the office of the Cumberland County Recorder of Deeds at Deed Book Volume 24, Page 489. A true and correct copy of the Deed is attached hereto, marked Exhibit "1", and made a part hereof. 5. Between February 25, 1999 and March 18, 1999, Claimant, pursuant to an oral contract with Contractor, agreed to supply crane rentals and erecting towers, both manned and unmanned, for new construction at the premises above-described. True and correct copies of the invoices describing the labor and materials supplied by claimant 0:\91050628.G to said premises are attached hereto, marked Exhibits 112" through "61', and made a part hereof. 6. Claimant last supplied materials and labor to the above-described premises on or about March 18, 1999. 7. The materials and labor supplied were utilized in the new construction at the above described premises, and were free from defect and were in accordance with Claimant's Agreement with Contractor. 8. The total amount claimed by Claimant to be due is $9,710.46, together with interest from April 18, 1999 for materials and labor provided in connection with the agreement to provide construction materials to Contractor upon the premises above described. 9. Formal notice of claimant's intention to file a Mechanic's Lien Claim was mailed to owner on May 27, 1999 by certified mail, return receipt requested, and served upon owner on or about June 1, 1999. A true and correct copy of Claimant's formal notices of intention to file Mechanic's Lien Claim, dated May 27, 1999, and the receipt for certified mail and domestic return receipt evidencing service of said Notice are attached hereto, collectively marked Exhibit "7", and made a part hereof. 0:\91050628.G -_- _/ 10.
