The Petitioner was divorced from Ronald Bowermaster on June 8, 1999 by decree of this Honorable Court docketed to No. 99-2538 Civil Term. 1 6. Following her divorce from Mr. Bowermaster, the Petitioner retook her maiden name, Heishman. 7. The legal father of Elizabeth Kaycee Bowermaster is James C. Nicholson, who resides at 91 Smith Road, Gardners, Cumberland County, Pennsylvania. 8. Paternity of Mr. Nicholson was established by order of this Honorable Court docketed to 525 Support 1992. 9. Elizabeth Kaycee Bowermaster does not now, nor has she ever, gone by the name of Nicholson. 10. The Petitioner has never held her daughter out as being a child of Ronald Bowermaster. 11. Ronald Bowermaster has never had contact with Elizabeth Kaycee Bowermaster since her birth, and no parent-child relationship has ever been established between them. 12. It would be in the best interest of Elizabeth Kaycee Bowermastei to have her name changed to Elizabeth Kaycee Heishman because: A. The mother and child desire to have the same last name. B. The child has no relationship with Ronald Bowermaster and does not desire to have his name. 13. There are no judgments or decrees of like character of record against Elizabeth Kaycee Bowermaster. WHEREFORE, the Petitioner requests this Honorable Court, pursuant to the Act of December 16, 1982, P.L. 1309, No. 295, §2, 54 Pa. C.S. §702 to do the following: A. Fix a hearing on this Petition; B. Direct that notice be given of this Petition and hearing; and C. Enter a decree changing the name of Elizabeth Kaycee Bowermaster to Elizabeth Kaycee Heishman.
