PETITION FOR NAME CHANGE
TO THE HONORABLE, THE JUDGES OF SAID COURT:
AND NOW COMES, the Petitioner, Jeffery Alan Barrus a/k/a Jeffery Alan Manatt by and
through his legal counsel, Coyne and Coyne, P.C., and avers the following in support of his Petition to
Change his legal name to Jeffery Alan Manatt:
1. Petitioner, Jeffery Alan Ban'us, is an adult suijuris, and resides at 713 Erford Road,
Camp Hill, Cumberland County, Pennsylvania.
2. Petitioner was born on November 26, 1970 in Los Angeles County, California and given
the legal name of Jeffery Alan Ban'us. A copy of the Birth Certificate is attached and marked as Exhibit
"A";
3. Since Petitioner's birth, his mother and Jeffery Alan Manatt, the Petitioner's stepfather,
raised Petitioner. A copy of his Mother's Affidavit is attached and marked as Exhibit "B";
Since the age of four months until the present time, Petitioner has been known as Jeffery
Alan Manatt;

Petitioner served in the military under the name Jeffery Alan Manatt;
Petitioner has resided at 713 Erford Road, Camp Hill, Cumberland County, Pennsylvania
during the three years preceding this Petition and prior thereto, Petitioner resided at 12 Marshall Drive,
Apt. 21, Camp Hill, Cumberland County, Pennsylvania;
7. Petitioner's Pennsylvania Driver's License is issued in the name of Jeffery Alan Manatt
and a copy of which is attached as Exhibit "C".
8. There are no judgments or adverse decrees of like character of record against Petitioner.
WHEREFORE, Petitioner respectfully requests that pursuant to 54 Pa. C.S. §701, et seq., this
Honorable Court enter a Decree changing the Petitioner's name from Jeffery Alan Barrus to Jeffery Alan
Manatt.
