Mother and Father shall have shared legal custody of the child/ehtk?dren, meaning that they shall share the legal right to make major decisions affecting the best interest of the child/cam and all of the major matters related to the child/ohil,dren, including, but not limited to, medical, religious and educational decisions. 2. Mother/Father shall have shared physical custody of the child/ meaning that the Mother and Father shall have 17 approximately equal physical possession and control of the child/old ldW. 1. HOLIDAYS OTHER THAN THANKSGIVING AND CHRISTMAS (Initial choice 1 or 2 or leave blank) 1. Holidays shall be defined as: (Initial holidays that you agree to alternate.) New Years Day Easter Sunday Memorial Day Independence Day (Fourth of July) Labor Day During even numbered years, Mother/Father shall have the child/children on New Years Day, Memorial Day and Labor Day and the other parent shall have the child/children on Easter Sunday 18 and Irn}ependence Day from .m. through .m. (Time) (Time) During odd numbered years, the other parent shall have the child/children on New Years Day, Memorial Day and Labor Day at the above times and Mother/Father shall have the child/children on Easter Sunday and Independence Day at the same times. 2. SPLIT Holidays shall be defined as: (initial holidays that you agree to split and not alternate.) New Years Day Easter Sunday Memorial Day Independence Day (Fourth of July) Labor Day Holidays shall be split between the parents with the parent that does not have primary physical custody having the child/children beginning at .m. on the day of the (Time) holiday through m. on the same day. ALTERNATING The parents hall alternate Thanksgiving holiday with Mother/Father having the child/children on even numbered years and the other parent having the child/children on odd numbered years. 2. SPLIT The parents shall split the Thanksgiving holiday with MGm n/FATHER having the child/children from the beginning of the Thanksgiving holiday through Day of Week at .m. and the other parent having the child/children (Time) from that time through the end of the Thanksgiving holiday. g. CHRISTMAS Christmas holiday (Initial if agreed) shall be defined as beginning on December at M. (Date) (Time) ending on at _.m. each year. (Date) (Time) (Initial choice 1,2, or 3 or leave blank) 1. Split Mother/Father shall have the child/children from the beginning of the Christmas holiday through December at _.m. and the other parent (Date) (Time) shall have the child/children from that time to the end of the Christmas holiday in even/odd numbered years. In the other years, the other parent shall have the child/children from the beginning of the Christmas holiday through the above time, and 20 custody shall then be transferred at the above time to Mother/Father. 2. ALTERNATING Mother/Father shall have the child/children during the Christmas holiday in even/odd years. In the other years, the other parent shall have the child/children during the Christmas holiday at the same times. 3. FIXED Mother/Father shall each year have the child/children from the beginning of the Christmas holiday through December at .m., and the (Date) (Time) other parent shall each year have the child/children from that time to the and of the Christmas holiday. h. SUZCM VACATION (Fill this in if the summer schedule will differ from the school year schedule on a regular basis or if you wish to have the right to take the child (ran) on a vacation trip.) The mother shall (Initial if agreed) shall have the child/children for weeks during the summer (Number) vacation. Father shall have the child/children for weeks during the summer vacation. The summer vacation is as follows: i. MISCELLANEOUS The parents also agree (Initial if agreed) to the following: J. Neither parent shall do anything which may estrange the child' from the other parent or injure the opinion of the child as to the other parent, or which may hamper the free and natural development of the child's love and affection for the other parent. 6. Neither parent shall consume alcoholic beverages to the point of intoxication or consume any unprescribed controlled substances (drugs) while the child/children is in their care. 7. Each parent agrees to provide the other parent with an address and telephone number through which the parent may be contacted during periods of custody of the child/children. Each parent further agrees to inform the other parent of any medical emergencies that arise while the child/children are in that parent's care. 8. The failure of either parent to insist on strict compliance with this agreement shall not prevent the parent from insisting on strict compliance in the future. 9. The parents agree and desire that this agreement be made an Order of Court of the Court of Common Pleas of Cumberland County, Pennsylvania.
