Petitioner currently resides at 839 Acri Road, Mechanicsburg, Cumberland County, Pennsylvania. 3. Petitioner has resided at her current address for the last nine (9) years. 4. Petitioner has had a long standing desire to have her name changed to include Sanscharowskiy, which is the surname of her step-father. 5. Petitioner has an extremely close relationship with her step-father who has been her father figure for the majority of her life. 6. Petitioner seeks the issuance of a Decree effectuating the desired change to acknowledge her step-father's important role in her life and to be recognized as part of his family. Petitioner is not seeking this change of name to avoid creditors or as a result of any prior criminal record.
