consideration of the attached Custody Conciliation Report, it is ordered d directed as follows:
1. The parties shall make arrangements for the Child and the Father to engage in therapeutic
family counseling with a counselor to be selected by agreement between the parties. The purpose of
the counseling shall be to address issues which have arisen in the Father-Daughter relationship and to
facilitate resumption of regular ongoing contact between the Father and the Child. Within ten (10)
days of the date of this Order, the Mother shall select a counselor as discussed at the custody
conciliation conference, and shall contact the counselor's office to schedule the initial appointment for
Nicole. The Father shall schedule his initial appointment with the counselor promptly upon returning
from vacation. The parties shall cooperate in scheduling all individual and joint sessions as
recommended by the counselor.
2. Within six (6) months of the date of this Order, counsel for either party may contact the
conciliator to schedule an additional custody conference, if necessary.

3. The parties agreed to entry of an Order in the form as attached.
