For sewer services rendered from 06/01/0Z to 06/01/03. ,Said services were rendered
pursuant to ordinance. Attached hereto is a statement of the kind end character of the
services so rendered and the p~'ice charged therefor, said statement being a true copy of
the owner's account with Hampden Township which sum of $78Z.72 was duly assessed
against the hereinabove-named owner and the premises on 06/0~/03 and for which sum,
with interest, penalties and costs, a lien is claimed against the above-described premises in
accordance with the Act of May 16, 1923, P.L. 207 (§3 P.S. Section 710~, et seq.), as
amended and supplemented, and the Act of May 2, 194§, P.L 38;> (§3 P.S. Section 301,
et seq), as amended and supplemented.
