AND NOW, this QT4&_ day of September, A.D. 1999, comes the
Petitioner, Cary James Ney, 11, by his Counsel. Clarence B. Turns, Jr., Esquire,
and presents his Petition for Change of Name upon a cause whereof the
following is a statement.
1. The Petitioner is Cary James Ney, 11, who is an adult person and who
resides at 2202 Fenwick Avenue, Mechanicsburg, Cumberland County,
Pennsylvania 17055.
2. The Petitioner was born on May 22, 1962, in the Borough of Lykens,
Dauphin County, Pennsylvania, and presently is 37 years of age. The
Petitioner's Social Security Number is 209-58-4451.
3. The Petitioner has resided at the following addresses for the following{
periods of time during the rive years immediately preceding the filing of this
petition, to wit:

6. The Petitioner has no criminal record.
7. The Petitioner desires to change his name to : "CARY JAMES KLOUSER".
8. The Petitioner desires to change his name as aforesaid for the reasons
that, although the Petitioner was given the name "Cary James Ney, II at birth--
as a result of the marriage of his mother, Susie Ann Ney [Behney] and his
father, Cary James Ney, I-the Petitioner's parents divorced when he was
approximately three years of age. Subsequently--after his mother married
Marlin H. Klouser a few years after the aforesaid divorce-the Petitioner began
to use his step-father's last name. The Petitioner has used the last name
"Klouser" consistently since he was 16 years of age. He has used said last name
on his driver's license and motor vehicle registration papers; on his employment
papers, Social Security registration and his tax returns; on his school records;
on his lease agreements and other contracts; and on his bank and other
financial accounts. The Petitioner is known as Cary James Klouser to everyone
who knows him. And all of the Petitioner's debts, credit cards, credit accounts,
bank loans and other financial obligations have been incurred in the name
"Cary James Klouser".

10. Neither the filing of this petition nor the granting of the requested change
of name will result in any concealment or fraud of any nature or in any
confusion of the Petitioner with any other person having a similar name. Neither
the filing of this petition nor the granting of the requested relief will result in any
injury to any person.
11. To the contrary, the filing of this petition and the granting of the relief
requested will result in the Petitioner's legal name becoming the name in which
the Petitioner has been known to everyone and in which the Petitioner has
transacted his business and affairs since he was 16 years of age.
12. The Petitioner has contracted no debts or obligations in the name "Cary
James Ney, II and no credit has been extended to him in that name. There are
no judgments, decrees, liens, encumbrances or other matters of record entered
against the Petition in the name "Cary James Ney, II.
13. The Petitioner has not incurred a criminal record in the name "Cary
James Ney, II" nor in any other name. There are no criminal convictions or
other criminal matters of record entered against the Petitioner in the name
"Cary James Ney, II".

WHEREFORE, the Petitioner prays that this Honorable Court, pursuant
to the provisions of the Act of June 18, 1998, P.L. 638, No. 83, Section No. 1, as
amended, 54 Pa.C.S. Section 701, et seq. will enter an order:

[B] Ordering and directing that notice of the filing of this petition
and of the said hearing be given as required.
