the best interest of the child, it is ORDERED and DIRECTED that the minor child, Logan
Tyler Mulholland, bom May 27, 1997 shall hereafter be known as LOGAN TYLER
CHETAITIS.

PETITION FOR CHANGE j NAME
1. Logan Tyler Mulholland is a minor individual with a date of birth of May 27,
1997.
2. Jennifer Chetaitis, natural mother, is an adult individual currently residing
at 6039 William Drive, Mechanicsburg, Cumberland County, Pennsylvania.
3. Thomas and Carole Chetaitis, maternal grandparents, are adult individuals
currently residing at 6039 William Drive, Mechanicsburg, Cumberland County,
Pennsylvania.
4. In August 1997, the natural mother separated from the natural father,
Mark P. Mulholland and moved to her parent's home referenced above.
5. The natural father, Mark P. Mulholland has not seen the child or had any
contact with the child since that date.
6. Following the separation, the mother petitioned for custody which was
granted by the Honorable George E. Hoffer on December 8, 1997 and captioned to 97-
4157 Civil Term after the father failed to appear. A copy of the Court's Order is
attached hereto and made a part hereof.
7. Thereafter, on December 21, 1998 the natural mother granted primary
physical and legal custody to her parents and the maternal grandparents, Thomas and
Carole Chetaitis. A copy of the Court Order signed by the Honorable Kevin A. Hess is
attached hereto and made a part hereof.
8. The Defendant, Mark Patrick Mulholland, had been residing at an address
in Pottstown, Pennsylvania, 117 North Hanover Street, Pottstown, PA 19464.
9. The Defendant had a Court Order of support by the Court of Common
Pleas of Cumberland County, however he failed to pay support and was ultimately
arrested and placed in prison for this violation.
10. After posting bond, the Defendant was released from prison and ultimately
the Plaintiff, Jennifer Chetaitis was notified that the father had moved to Phoenix,
Arizona. A copy of the various correspondence from Frank Goshom, Cumberland
County Domestic Relations Officer is attached hereto and made a part hereof.
11. Ever since May 6, 1999, the last correspondence from the Domestic
Relations Office, the Plaintiff Jennifer Chetaitis has not received any support.
12. The Petitioners, natural mother and maternal grandparents request a
change of name as it will be in the child's best interest, the child is living with the
Chetaitis family and is known to the community as a Chetaitis, and continuing to keep
the name of an absent, non-interested father, who is in violation of Court Orders, would
not be in the child's interest, would be embarrassing and disturbing to the child.
WHEREFORE, for all the above reasons, the Petitioners request that this Court
grant their Petition and change the name of Logan Tyler Mulholland to Logan Tyler
Chetaitis.
