Petitioner now desires to take the last name of her step-father of ten (10) years, "Miller," to represent the actual familial relationship she has maintained with him during that period. 5. Petitioner resided in the home of her natural mother and step-father, whom her natural mother married in 1988, from 1979, when Petitioner was only five (5) years of age, until December 1998, when she took up her own residence in Mechanicsburg as noted above. 6. There are no judgments pending against Petitioner in Cumberland County, Pennsylvania or in any other jurisdiction. 7. Along with this Petition, Petitioner has submitted an additional copy of the Petition and a set of her fingerprints taken by Officer James R. Sadler of the Silver Spring Township Police Department for submission to the Pennsylvania State Police.
