PETITION FOR NAME CHANGE
AND NOW, this 28" day of September, 1999, comes Petitioner, Kahyle Drew Star, by and
through his attorney, Michael J. Hanft, Esquire, and files the following Petition, and in support
thereof, avers the following:
1. Petitioner, Kahyle Drew Star, is an adult individual residing at 166 Kline Road,
Shippensburg, Cumberland County, Pennsylvania.
2. Petitioner has been a resident of Cumberland County, Pennsylvania for at least six
(6) months previous to the filing of this Petition.
3. Petitioner desires to change his name from Kahyle Drew Star to Kahyle Star LeMay.
4. Petitioner was bom on October 2, 1977, in Shanon County, South Dakota. A copy
of Petitioner's birth certificate is attached hereto as Exhibit "A" and by reference incorporated herein
and made a part hereof.
5. Petitioner does not wish to change his name of Kahyle Drew Starr for any illegal
purpose or to avoid any obligation.
6. Petitioner, Kahyle Drew Star's Social Security Number is 504-78-4252.
WHEREFORE, Pclitioner, Kahyle Drew Star, respectfully requests this Honorable Court to
issue an Order changing Kohyle Drew Star's name to Kahyle Star LeMay.

of South Dakota permission to amend my name on my birth certificate from Kahyle Drew
Star to Kahyle Star LeMay. In 1999, I received a court ordered document stating my name
change,however,the South Dakota Vital Records Office is specifically requesting for one
that grants permission to do so. I am currently in the process of being enrolled in the
Oglala Sioux Tribe of South Dakota, but they have requested this document in order to
