PETITION FOR CHANGE OF SURNAME
Patti Louise Humes respectfully sets forth the following:
I. Petitioner is Patti Louise Humes, an adult person, who lives at #43 SME
Shippensburg, Cumberland County, Pennsylvania.
2. Petitioner married Ronald Glenn Slozat on December 31, 1988. A copy of her
Certificate of Marriage is attached as Exhibit A.
3. Petitioner desires to change her surname to Slozat while retaining Louise as a
middle name.
4. Petitioner has no child whose name may be affected by these proceedings.
5. Petitioner desires to have the same surname as her husband and does not seek to
defraud creditors.
Wherefore, Petitioner Patti Louise Humes requests that this honorable court order
that her surname be changed to Slozat so that her full name will be Patti Louise
Slozat.

A NAME CHANGE FOR A PERSON CONVICTED OF A FELONY HAS BEEN ORDERED. The
Pennsylvania State Police, upon receipt of this notice, shall include the change of name information in the
Central Repository as provided for in 18 PA C.S., Chapter 91 (relating to criminal history record
information).
