MOTHER resides at 275 East Crestwood Drive, Camp Hill, Cumberland County, Pennsylvania. 2. FATHER is John S. Youngblood, Defendant in the above-referenced matter. FATHER resides at 7073 - Lot 72, Carlisle Pike, Carlisle, Cumberland County, Pennsylvania. 3. MOTHER and FATHER are the natural parents of two (2) minor children, Stephanie Lynn, age 6, whose date of birth is October 21, 1994; and Jessica Suzanne, age 4, whose date of birth is May 21, 1997. 4. The parties shall share legal custody of their minor children. Legal custody shall be defined as the right to make important decisions regarding their children's lives, including but not limited to medical decisions, religious decisions, educational decisions, and extracurricular activity decisions. 5. The parties shall share physical custody of their minor children, with the children to reside on a primary basis with MOTHER. 6. FATHER shall have partial physical custody of the children at any time that is mutually agreeable to the parties. 7. The parties shall continue to share all transportation in connection with these custodial exchanges, with all exchanges to occur at a mutually acceptable public place, such as a supermarket or restaurant parking lot. 8. All holidays shall be shared by the parties pursuant to their mutually acceptable agreement 9. Except in the case of an emergency, in the event that either party wishes to take the children out of the state of Pennsylvania, that parent shall notify the other parent at least thirty (30) days prior to any such trip, and provide telephone numbers and addresses where the children can be reached. 10. MOTHER and FATHER acknowledge that they each play an important role in the lives of their children and agree to make every effort to cooperate with each other in raising their children. 11. MOTHER acknowledges that she is able to care for the children without financial contribution from FATHER at this time, and therefore no child support is being paid. 12.
