and step father were married and her mother took the name Mary Catherine Rogers.
3. ELI%ABETI-1 ANNE ROGERS wishes to retake her mothers maiden name.
4. ELI7_ABE"rl-I ANNE ROGERS has resided at 21 Rolo Court, Mechanicsburg,
Cumberland County, Pennsylvania since November 1995. Prior to November 1995,
ELIZABETH ANNE ROGERS resided at 2350 Boas Street, I larrisburg, Dauphin County since
1990.
5. There are no judgments or decrees of like character or record against the petitioner.
WI IEREFORE, Petitioner requests the court, pursuant to the Act of December 16, 1982,
P.L. 1309, No. 295, §2, 54 Pa. Cons. Stat. §702, to:
