PETITION FOR CHANGE OF NAME
NOW COMES Your Petitioner, SUSAN FRANCES FABIANKOVITZ, by her
attorney, Charles E. Petrie, and respectfully represents as follows:
1. That Your Petitioner is SUSAN FRANCES FABIANKOVITZ, an adult
individual currently residing at 421 Belle Vista Drive, P.O. Box 263,
Summerdale, County of Cumberland, Pennsylvania.
2. That Your Petitioner desires to change her name to SUSAN FRANCES
MUNDIS. Your Petitioner currently is carrying the name of her former
husband. Your Petitioner's fiancee, Karl James Mundis, died on September 26,
1999, to whom she was to marry upon the completion of his divorce. Your
petitioner desires to carry the name of her deceased fiancee to honor his
memory.
3. That Your Petitioner has resided at 421 Belle Vista Drive, P.O. Box
263, Summerdale, Pennsylvania, since 1952.
WHEREFORE, Your Petitioner respectfully requests that Your Honorable
Court pursuant to the Act of April 18, 1923, P.L. 75, Section 2, as amended, 54
P.S. Section 2, to:
(a) fix a hearing date for this Petition; and
(b) direct that notice be given of the filing of this Petition and of the
hearing.

( ) A criminal record was revealed containing felony convictions. IN ACCORDANCE WITH THE
NAME CHANGE ACT, THE COURT SHALL NOTIFY THE PENNSYLVANIA STATE POLICE
WHEN A NAME CHANGE FOR A PERSON CONVICTED OF A FELONY HAS BEEN
ORDERED. The Pennsylvania State Police, upon receipt of this notice, shall include the change
of name information in the Central Repository as provided for in 18 PA C. S., Chapter 91 (relating
to criminal history record information).
