The said decedent, who resided at Claimant's facility at the time of her passing, located at
770 Poplar Church Road, Camp Hill, Pennsylvania 17011, died on February 10th of2007.

West Limekiln Road, New Cumberland, P A 17070 on April 11 th of2007.
