NO. 99 - PETITION FOR CHANGE OF NAME Petitioner, KENNETH MICHAEL WEAVER, through his attorneys, FLOWER, FLOWER & LINDSAY, requests that this Court order a change of name for him pursuant to 54 Pa.C.S. § 701 et.seq., and in support thereof represents as follows: Petitioner, KENNETH MICHAEL WEAVER, is an adult individual, age 18 years, residing at 16 Run Road, Carlisle, Cumberland County, Pennsylvania. 2. Petitioner was born October 1, 1981, in Lehigh County, Pennsylvania. 3. At the time of his birth, Petitioner's mother was unable to take care of him, and his maternal grandmother, Barbara Ann Beard, raised him as her son, although without the benefit of a formal adoption, and throughout his life, he has been known as KENNETH MICHAEL BEARD. 4. Neither Petitioner's natural mother nor his natural father, have been a significant part of his life. He only learned in the last two years or so that his formal name was KENNETH MICHAEL WEAVER. 5. All of Petitioners school records reflect the name KENNETH MICHAEL BEARD. His birth certificate and social security number, however, reflect the name KENNETH MICHAEL WEAVER. After discussing this matter with the local social security representatives, he finds that he is not able to obtain a social security number for the name KENNETH MICHAEL BEARD without a formal Order changing his name. 6. Petitioner also believes that unless he is granted an Order formally changing his name there will be complications relating to employment, driver's licensing and college records, which he would prefer to avoid. 7. There are no judgments or decrees of like character of record against Petitioner, and this Petition is not in any intended to avoid just debts of creditors.
