AMENDED PETITION FOR CHANGE OF NAME
TO THE HONORABLE, THE JUDGES OF SAID COURT:
The Petition of KERSTIN NICHOLE SHERIFF, a minor, BY HER MOTHER,
LORETTA SHERIFF, and by their attorney, James J. Kayer, Esquire, respectfully represents:
1. Petitioner desires to change her daughter's name for the following reasons: Mother
has primary physical custody and wants to change the name so that it is consistent with her own.
2. That petitioner resides at 313 Walnut Dale Road, Shippensburg, PA 17257, and has
been a resident of the Commonwealth of Pennsylvania for a period of at least six months.
3. The child Kerstin Nichole Sheriff was born May 27, 1997 and her natural parents are
Loretta Sheriff, and Brian Jesse Evans, an adult individual presently incarcerated at B Block,
Section 304, MTC, 954 4'h Street, Baltimore, Maryland 21202-4256.
4. Petitioner and her counsel have advised the child's father of her intention to file this
petition, and the father has failed to consent to the proposed change of name.
5. That there are no judgements or decrees of like character of record against the
petitioner.
6. Petitioner does not wish to change the name of her daughter, Kerstin Nichole Evans
for any illegal purpose or to avoid obligation.
WHEREFORE, Petitioner requests the Court, pursuant to the Act of December 16, 1982,
P.L. 1309, No. 295, Section 2, 54 Pa. Cons. Stat. Section 702. to:
(a) Fix a hearing on this Petition;

(b) Direct that notice of this Petition and hearing be published; and
(c) Enter a decree changing the Petitioner's name from KERSTIN NICHOLE EVANS to
KERSTIN NICHOLE SHERIFF.
