Petitioner is JESSICA KOSER, an adult individual and daughter of the alleged incapacitated person, residing at 631 Range End Road, Dillsburg, York County, Pennsylvania 17019. 2. The alleged incompetent is PATRICIA LOGAN, born June 25,1951 (age 55 years), whose permanent residence address is 1316 Zimmerman Road, Carlisle, Cumberland County, Pennsylvania 17015. 3. The alleged incapacitated person became incapacitated and has been unable to handle her own affairs on or about April 11 , 2007. Her spouse, Steven C. Logan, died on April 13, 2006. 4. The alleged incapacitated person's incapacity is documented in a competency affidavit prepared by her treating physician, D. Patrick Bryant, M.D., which statement is incorporated herein by reference and attached hereto as Exhibit "A". 5. The petitioner, JESSICA KOSER, is the alleged incapacitated person's sole next-of-kin and she has attached her consent to the proposed guardianship at Exhibit "B". 6. Petitioner has no knowledge of any other Court within this Commonwealth, which has appointed a guardian for the alleged incapacitated person. j-- i C5 U1 &'" 7. Prior to the onset of the alleged incapacity, the alleged incapacitated person had no income but possesses a home and other resources, the values of which have not yet been ascertained by the petitioner. 8. Petitioner has no interest adverse to the alleged incapacitated person and has agreed to act as guardian if this Honorable Court shall so appoint. WHEREFORE, p-etitioner respectfully requests this Honorable Court issue a rule upon Patricia Logan, the alleged incapacitated person, with notice thereof to be given to her next-of-kin, and to such other persons as this Court may direct, to show cause why she should not be adjudged an incapacitated person and petitioner be appointed guardian of her person and estate.
